The undersigned states and affirms that he or ahe is desirous of constructing, ren vatiag, addiag to or reroofiag his or her owa domicile, that he or she act ally occupies, or will occupy by said domicile, and s�sne is not for rent, lease or sale. That he or she shall comply with the following conditioas: 1. That the owner and he or she aloae shall act as the builder for all phaees of construction. " 2. That the owner will comply with all provisioas of the City of Zephyrhills ordinances and codes pertiaent to the buildiag. 3. That ia the eveat various phases of coastructioa are subcontracted, he will engage oaly properly licensed subcoatractors aad will persoaally supervise such work. 4. That in the eveat the Buildiag Inspector shall require correctioas to be made, the owaer will assume full responsibility to iasure they are made, aad upon � completioa will call for a reiaspectioa before proceeding with the building. 5. That the owner shall assume full responsibility for the conatruction aad will aot expect supervisioa of his work from the City of Zephyrhills Buildiag . Department. 6. That prior to fiaal iaspection aay additional fees, iacluding reinspection fees, must be paid ia full. A writtea request from this office ahall constitute an official notice to pay additioaal fees. 7. That the owaer shall comply with all the safety codes issued by the Florida Industrial Commissioa. 9. State law requires construction to be done by liceased contractors. You have applied for a permit uader aa exemption to that law. The exemption allows you, as the owaer of your property, to act as your owa contractor with certain restrictions even though you do aot have a licease. You must provide $irect oasite supervision of the coastruction yourself. You may build or improve a one-fasnily or two-family resideace or a farm outbuildiag. You may also build _ or improve a commercial building, provided your costs do not exceed $75,000. The buildiag or resideace must be for your own use or occupancy. It may aot be built or substantially improved for sale or lease. If you sell or lease a building you have built or substaatially improved yourself withia 1 year after the coast'ruction is complete, the law will presume that you built or substantially improved if for sale or lease, which is a violatioa of this exemption. You may aot hire an ualiceased persoa to act as your coatractor or to supervise people working oa your buildiag. It is your responaibility to make sure that people employed by you have liceases required by state law and by county or municipal licensiag ordiaaaces. You may aot delegate the responsibility for aupervisiag work to a liceased contractor who is aot licensed to perform the work being doae. Any persoa workiag oa your buildiag who is aot liceased must work uader your direct supervisioa aad must be employed by you, which meaas that you must deduct F.I.C.A. and withholdiag tax and provide workers' compensation for that employee, all as prescribed by law.
