DocuSign Envelope ID:E5512092-46F9-4E63-AB5E-BAD7CA81408F Agreement#SLO52 AMENDMENT TWO TO GRANT AGREEMENT BETWEEN THE DEPARTMENT OF ECONOMIC OPPORTUNITY AND THE CITY OF ZEPHYRHILLS On June 21, 2018,the State of Florida, Department of Economic Opportunity ("DEO"), and the City of Zephyrhills ("Grantee"), entered Into Grant Agreement SL052 ("Agreement"). DEO and Grantee are sometimes referred to herein individually as a"Party"and collectively as"the Parties." WHEREAS, Section D.of the Agreement provides that any amendment to the Agreement shall be in writing and executed by the Parties thereto;and WHEREAS,the Parties wish to amend the Agreement as set forth herein. NOW THEREFORE,the Parties hereby amend the Agreement as follows: 1. Section A,Agreement Period,is hereby deleted In Its entirety and replaced with the following: A. AGREEMENT PERIOD: This Agreement shall be effective as of July 1, 2018 (the "Effective Date")and shall continue until the earlier to occur of(a) December 31,2019 (the"Expiration Date")or(b)the date on which either Party terminates this Agreement (the`Termination Date"). The period of time between the Effective Date and Expiration Date or Termination Date Is the"Agreement Period". DEO shall not reimburse Grantee for any eligible costs (hereinafter referred to as "obligations") incurred after June 30, 2019, provided, however,that Pursuant to s. 216.301, Florida Statutes (FS.) any such obligations Grantee incurred prior to June 30,'2019, for which services, tasks, or deliverables are expected to be completed after June 30, 2019, may be eligible for reimbursement as long as these services, tasks, or deliverables are satisfactorily completed and comply with both s.216.302,F.S.and Section 6.1,invoice Submittal and Payment Schedule,of this Agreement,on or before September 20, 2019, as DEO shall determine,in DEO's sole and absolute discretion. 2. Anew unnumbered paragraph is hereby added to paragraph 5.1 of the Scope of Work,to read as follows: 5.1 Monthly: Starting October 1,2019, Grantee shall report on the progress of the Project monthly. The monthly report shall include a summary of project progress, including before and after photographs of the completed work, containing listing of goods received, percentage of completion of the project,and all additional reports which are required pursuant to this Agreement. The summary shall also Include any issues or events occurring which affect the ability of the Grantee to meet the terms of this Agreement. Monthly reports are due to DEO within 10 days after the end of each month,until the Project is complete. 3. All instances of"September 15, 2019" contained In Section 6.1 of the Scope of Work are hereby deleted and replaced with"September 20,2019." Page 1 of 2
