GENERAL NOTES AND SPECIFICATIONS
<br />1. Unless otherwise noted on the plans, all construction
<br />methods, workmanship, materials and testing shall conform to
<br />the "Florida Department of Transportation (FDOT)'s "Standard
<br />Specification for Road and Bridge Construction", its current
<br />supplements and standard index drawings, and any city,
<br />COUNTY, or any other agency's rules and regulations.
<br />2. Survey layout and testing shall be by the Contractor.
<br />3. All work within the right-of-way shall be coordinated
<br />with the CITY, County, and/or FDOT. All disturbed areas
<br />within the right-of-way shall be restored to the
<br />original condition or better in accordance with
<br />county, and/or FDOT standards.
<br />4. Locations, elevations and dimensions of existing
<br />utilities, structures, and other features are shown
<br />according to the best information available at the time
<br />of preparation of these plans. The contractor shall
<br />verify the locations, elevations and dimensions of
<br />existing utilities, structures and other features
<br />affecting this work prior to construction.
<br />5. The contractor shall use extreme caution in areas of
<br />buried utilities and shall provide at least 48 hours
<br />notice to the utility companies prior to construction
<br />to obtain field locations of existing underground
<br />utilities. The contractor shall protect and maintain
<br />operation of existing utilities during construction.
<br />6. The contractor shall check plans for conflicts and
<br />discrepancies prior to construction. The contractor
<br />shall notify the engineer of any conflicts before
<br />performing any work in the affected area.
<br />7. The Engineer shall be notified as to where existing
<br />utility services interfere with the proposed construction.
<br />8. The Contractor is responsible for repairing any damage
<br />to existing facilities, above or below ground, that may
<br />occur as a result of the work performed by the contractor.
<br />9. All underground utilities must be in place and tested
<br />or inspected prior to base and surface construction.
<br />10. Utilities installation shall conform to the requirements
<br />of the applicable federal, state and local regulations
<br />including but not limited to the Florida Department of
<br />Environmental Protection (FDEP), Department of Health
<br />(DOH), the Occupational Safety and Health Administration
<br />(OSHA) Standard and the city.
<br />11. It is the contractor's responsibility to become familiar
<br />with the permit and inspection requirements specified
<br />by the various governmental agencies and the engineer
<br />The contractor shall obtain all necessary permits prior
<br />to construction and schedule inspections according to
<br />agency instructions. All work performed shall comply with
<br />the regulations and ordinances of the various governmental
<br />agencies having jurisdiction over the work.
<br />12. At least three (3) working days prior to construction,
<br />the contractor shall notify the engineer and appropriate
<br />agencies and supply them with the contractor's name,
<br />starting date, projected construction schedule, all required
<br />shop drawings and other information as required. Any work
<br />performed prior to proper notifications may be subject to
<br />removal and replacement at the contractor's expense.
<br />13. The site shall be initially graded such that no offsite
<br />area will be adversely affected by storm water runoff.
<br />Runoff to the adjacent right-of-way shall be minimized. The
<br />existing drainage facilities within the right-of-way shall
<br />be protected at all times by the use of staked hay bales,
<br />temporary grading, revegetation, etc. Any disturbed area
<br />within the right-of-way shall be sodded. The contractor
<br />shall final grade and hand rake all exposed earthwork areas
<br />immediately prior to installation of sod. All areas shall be
<br />protected from erosion caused by storm water runoff by
<br />vegetation, staked hay bales, temporary grading, etc.
<br />14. Upon completion of construction activities, the storm
<br />water management system shall be cleaned of all debris,
<br />silts, limerock, etc.
<br />15. The contractor shall verify minimum separation between
<br />water and sanitary sewer systems (18" minimum vertical and
<br />10' minimum horizontal separations). Upon completion of
<br />construction, the contractor shall provide written
<br />verification documenting the minimum separation as
<br />constructed.
<br />16. Typical cover over utilities shall be 36".
<br />17. During construction and/or maintenance of this project,
<br />all safety regulations are to be enforced. The contractor
<br />or his representative shall be responsible for the control
<br />and safety of the traveling public and the safety of his
<br />personnel.
<br />18. The contractor shall comply with all Federal/State
<br />Occupational Safety and Health Act (OSHA) Standards and any
<br />other rules and regulations applicable to construction
<br />and/or maintenance activities in the State of Florida. The
<br />Contractor shall also comply with Chapter 442, Florida
<br />Statutes (Toxic Substances in the Workplace), and any
<br />CITY, COUNTY, or any other agency's rules and regulations
<br />regarding safety.
<br />19. All trenching and excavation operations MUST conform to
<br />the requirements stated in the new "Trench Safety Act"
<br />incorporated into OSHA standards 29 CFR 1926 Subpart P final
<br />rule and the State of Florida Trench Safety Act. The
<br />contractor shall recognize the OSHA safety standards, agree
<br />to abide by them, and identify the cost to comply.

<br />20. Further Contractor operations shall comply with 29CFR
<br />1910.252 and NFPA 51 B for cutting and welding procedures.
<br />21. All persons on (CITY, COUNTY OR STATE) property and
<br />in area where the noise level exceeds 85db, must
<br />wear hearing protection that complies with ANSI S3.19-74
<br />(ear muffs and/or approved ear plugs). This includes areas
<br />where noisy equipment is in use (i.e. jack hammers, electric
<br />or air drills, heavy equipment with open cabs, pipe cutting
<br />saws, etc.) and in the plant where posted.
<br />22. All work conducted in an elevated position shall comply
<br />with 29CFR 1910.269.
<br />23. All design application, installation, maintenance, removal
<br />of all traffic control devices, warning devices and barriers
<br />necessary to protect the public and workmen from hazards
<br />within the projects shall be in accordance with the minimum
<br />standards as set forth in the current edition of the State
<br />of Florida and Traffic Design Standards.
<br />24. All traffic control markings and devices shall conform
<br />to the provision set forth in the "Manual on Uniform
<br />Traffic Control Devices" prepared by the U.S. Department of
<br />Transportation, Federal Highway Administration.
<br />25. All Contractor owned or controlled vehicles and/or
<br />equipment which will be operated on or within ten (10) feet
<br />of the roadway shall be equipped with a minimum of one amber
<br />360 degree Class I warning device. This device must meet
<br />minimum standards for utility construction purposes, such as
<br />a minimum of 500,000 candlepower and visible from 360
<br />degrees of mounting. The warning device(s) must be in
<br />operation at all times that a vehicle/equipment if on the
<br />roadway or within the ten (10) feet of runoff area and not
<br />in a "normal" travel status.
<br />26. Area designated by the Owner shall be utilized for
<br />equipment storage and cover material staging. Materials and
<br />supplies shall be stored only in those areas approved by the
<br />owner.
<br />27. In regard to the inspection of the project, Southwest
<br />Florida Water Management District (SWFWMD) and the city
<br />inspectors shall have the same right of review and
<br />inspection as the Engineer of Record. Although it is
<br />appropriate for the Engineer of Record to resolve questions,
<br />diffic�ities, and disputes relative to interpretation of
<br />contract documents or construction; SWFWMD and the city
<br />reserve the right of final concurrence on such matters.
<br />28. The city's Engineering inspector shall be
<br />notified, and the Engineer's approval shall be
<br />obtained, on all proposed field changes, alterations or
<br />additions to the approved plans by the Contractor or
<br />the Engineer of Record prior to the implementation of such
<br />changes, alterations or additions.
<br />29. The contractor shall obtain a separate permit for
<br />any on -site burning.
<br />30. If during construction activities any evidence of historic
<br />resources including but not limited to aboriginal or
<br />historic pottery, prehistoric stone tools, bone or shell
<br />tools, historic trash pits, or historic building foundation,
<br />are discovered, work shall come to an immediate stop
<br />and the Florida Department of Historic Resources (State
<br />Historic Preservation Officer) and Pasco County shall
<br />be notified within two working days of the resources
<br />found on the site.
<br />31. All proposed signs must be applied for, approved and
<br />permitted on an individual basis apart from ultimately
<br />approved site plan. Approval of this site plan does
<br />not constitute approval of any signage.
<br />32. Handicap parking spaces will be Properly signed and
<br />and striped in accordance with Florida Statute 316, the
<br />Manual on Uniform Traffic Control Devices, or other
<br />applicable standards.
<br />33. The Engineer certifies that the site has been designed
<br />in accordance with the American Disability Act.
<br />34. All on -site parking spaces will be striped and signed in
<br />accordance with the Manual on Uniform Traffic Control
<br />Devices Latest Edition. Parking spaces, directional
<br />arrows, and stop bars shall be striped in white. It shall
<br />be the Owner/Developers responsibility to properly sign
<br />and stripe the site in accordance with applicable standards.
<br />35. All clear -site areas shall be kept free of any signage
<br />plantings, trees, etc. In excess of three and one-half
<br />feet in height.
<br />36. All structures including buffer walls, retaining walls,
<br />fencing, signage etc. require separate building permits.
<br />37. No irrigation system or landscaping shall be installed in any
<br />city right of way without the appropriate right
<br />of way permit.
<br />38. Upon completion of construction, the Owner/or Contractor
<br />will be require to supply one set of complete "As -Built".

<br />to the Engineer of Record.
