<br />A. NOTICE OF DEED RESTRICTIONS

<br />The undersigned understands that this permit may be subject t~ "deed restricti~ns" which may be m~re restrictive than City
<br />regulations. The undersigned assules resp~nsibility for compliance with any applicable deed restricti~ns.

<br />B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES

<br />If the ~wner has hired a c~ntract~r or contractors to undertake w~rk, they may be required t~ be licensed in acc~rdance with
<br />state and l~cal regulati~ns. If the contract~r is not licensed as required by law, both the owner and contractor ~ay be
<br />cited for a misdemeanor violation under state law. If the ~wner or intended contractor are uncertain as to what licensino
<br />requirements may apply for the intended work, they are advised to contact the City of Zephyrhills Building Departffient, (BI3)
<br />788-6611.

<br />Furthermore, if the owner has hired a contractor or contractors, he is advised to have the contractor(sl sign portions of the
<br />"Contractor Sections" ~f this application for which they will be responsible. If you, as the owner sign as the c~ntractor,
<br />y~u are indicating that y~u, rather than the c~ntract~r, are resp~nsible for the work. If the contractor wishes y~u t~ sign
<br />as contract~r that may be an indication that he is n~t pr~perly licensed and is not entitled to permitting privileges in the
<br />City of Zephyrhills.
