<br />state and loral regulations. If the contrartor is not lirensed as required by laN, both the owner and contrartor lay be
<br />cited for a lisdeleanor violation under state IaN. If the ONner or intended rontrartor are uncertain as to "hat licensing
<br />requirelents lay apply for the intended work, they are advised to rontart the City of Zephyrhills Building Departlent, (813)
<br />788-6611.

<br />Furtherlore, if the ONner has hired a contractor or contrartors, he is advised to have the contrartorls) sign portions of the
<br />.Contractor Sertions" of this application for which they will be responsible. If you, as the owner sign as the contrartor,
<br />you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wishes you to sign
<br />as rontractor that lay be an indication that he is not properly licensed and is not entitled to perlitting privileges in the
<br />City of Zephyrhills.

<br />installation has cOllenced prior to issuance of a perlit and that all work Nill be perfor.ed to leet standards of all laws
<br />regulating construction, City rodes, zoning regulations, and land develop.ent regulations in the juri~diction. I also
<br />rertify that I understand that the regulations of other governlental agencies lay apply to the intended work, and that it is

<br />set aside any provisions of the technical codes, nor shall issuance of a perlit prevent the Building Official fro. thereafter
<br />requiring a correction of errors in plans, construction, or vi~lations of any code. Every perlit issued shall becole invalid
<br />unless the Mork authorized by such perlit is cOllenced within six tonths of issuance, or if Nork authorized by the per.it is
<br />suspended or abandoned for a period of six .onths after the tite the work is totlenced. One 90 day extension of tile, .ay be
<br />allowed for the perlit with fee charge of $15.00. The extension'~hall be requested in Nriting to the Building Offirial. An
<br />approved inspertion lust be logged during each six lonth period, or the project Nill be considered abandoned.
