If the contractor wisbes you to sign <br />as contractor that lay be an indication that be is not properly licensed and is not entitled to perlltting privileges in the <br />City of Zepbyrhills. I certify that no work or <br />installation bas cOlleDced prior to issuance of a perlit and that all work will be perforted to l8et standards of all laws <br />regulating construction, City codes, loning regulations, and land developleDt regulations in the jurisdiction. Every perlit issued sball beCOle invalid <br />unless the work authorized by such perlit is cOlIBnced within six IOnths of issuance, or if wort authorized by the perlit is <br />suspended or abandoned for a period of sillOnths after the tile the work is co.enced. One 90 day extension of tite, lay be <br />allowed for the perlit with fee charge of $15.00. Tbe extension sball be requested in writing to the Building Official.
