Permitted construction or alteration of public water supply <br />systems must be supervised during construction by a professional <br />engineer registered in the state of Florida. The permittee must <br />retain the service of an engineer re9istered in the state to <br />observe that construction of the project is in accordance with <br />the engineering plans and specifications as submitted in support <br />of the application for this permit. <br /> <br />4. Permitted construction or alteration of a public water system <br />may not be placed into service until a letter of clearance has <br />been issued by this Department. <br /> <br />5. A letter of clearance may be issued upon receipt of the <br />following items: <br /> <br />a. Complete and fully executed form "Request for a Letter of <br />Release to Place Water Supply System into Service~, DEP Form <br />62-555.910(9); <br /> <br />b. Copies of satisfactory bacteriological analyses of water <br />taken on two consecutive days from the point of connection with <br />the existing system and representative points within the <br />distribution. (chlorine residual readings must be recorded for <br />each water sample or the analysis will not be acceptable for use <br />in clearance of the system); and <br /> <br />c. Copy of a satisfactory pressure test of the distribution <br />system performed in accordance with AWWA Standards; and <br /> <br />6. The permittee must instruct the engineer of record to request <br />system clearance from the Department within thirty (30) days of <br />completion of construction, testing and disinfect1ng the system. <br />Bacteriological samples for clearance of water lines will not be <br />accepted for clearance if the analysis date for any of the <br />samples is over thirty days before the date that clearance is <br />requested. The bacteriological well survey will not be <br />acceptable for clearance of the well if the analysis date for the <br />last sample in the series is over thirty days before the date <br />that clearance is requested. <br /> <br />7. ,If historical or archaeol~~ical artifacts, such as Indian <br />canoes, are discovered at any time within the project site the <br />permittee shall immediately notify the district office and the <br />Bureau of Historic Preservation, Division of ArChives, History <br />and Records Management, R.A. Gray Building, Tallahassee, Florida <br />32301, Telephone number (904) 487-2073. <br /> <br />8.
