<br />cited for a lisdeleanor violati9n under state law. If the owner or intended contractor are uncertain as to wbat licensing
<br />requireaents 8aY apply for the intended work, they are advised to contact the City of Zepbyrbills Building DepartJent, (813)
<br />788-6611.

<br />FurtherlOre, if tbe owner has bired a contractor or contractors, be is advised to have tbe contractor(s) sign portions of tbe
<br />.Contractor Sections" of this application for whicb they will be responsible. If you, as the owner sign as the contractor,
<br />you are indicating that you, rather than the contractor, are responsible for the work. If tbe contractor wisbes you to sign
<br />as contractor that Jay be an indication that be is not properly licensed and is not entitled to perlitting priVileges in the
<br />City of Zepbfrbills.

<br />I certiff that all the inforlation in tbis application is accurate and that all work will be done in cOlpliance with all
<br />applicable laws regulating construction, zoning, and land developleDt.

<br />Application is bereby lade to obtain a perlit to do work and installation as indicated. I certiff that no work or
<br />installation bas cOJJenced prior to issuance of a perlit and tbat all wort will be perforaed to leet standards of all laws
<br />regulating construction, City codes, zoning regulations, and land developlent regulations in the jurisdiction. I also
<br />certify that I understand that the regulations of other goveDllental agencies JaY apply to the intended worl, and that it is

<br />Florida prior to perlit issuance.

<br />A perlit issued shall be construed to be a license to proceed witb tbe work and not as authoritf to violate, cancel alter, or
<br />set aside any provisions of tbe tecbnical codes, nor shall issuance of a perlit prevent the Building Official frOl thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Everf peIlit issued shall becote invalid
<br />unless tbe work authorized by such perlit is cOllenced within six IOnths of issuance, or if work authorized bf the perlit is
<br />suspended or abandoned for a period of sil 8Dntbs after the tile the work is c~ced. One 90 day l!Itension of tile, Jay be
<br />allowed for tbe perlit with fee charge of $15.00. Tbe l!Itension sball be requested in writing to the Building Official. An
<br />approved inspection lust be logged during each sillOnth period, or the project will be considered abandoned.
