<br />you are indicating that you, rather than the contractor, are responsible for the wort. If the ccmtractor wilhes you to sign
<br />as ccmtractor that .y be an indication that he is not properly licensed and is not entitled to perlittiDg privileges in the
<br />City of Zephyrhilll.

<br />suspended or abandoned for a period of slllOlltb8 after the tile the IOrk is ~ced. One 90 day atension of tile, IIilY be
<br />allowed for the peIlit with fee charge of $15.00. fbe atension shall be requested in writing to the Building Official. An
<br />approved inspection lUSt be logged during eaeb slllODth period, or the project will be considered abandoned.
