<br />state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor JaY be
<br />cited for a lisdeaeanor violation under state law. If the owner or intended contractor are uncertain as to wbat licensing
<br />requireaents laY apply for the intended work, they are advised to contact the City of Zepbyrbills Building Departaent, (813)
<br />788-6611.

<br />Furtheraore, if the owner bas bired a contractor or contractors, be is advised to bave the contractor(s) sign portions of the
<br />IContractor Sectional of this application for wbieb they will be responsible. If you, as the owner sign as the contractor,
<br />you are indicating that you, rather than the contractor, are responsible for the wort. If the contractor wisbes you to sign
<br />as contractor that laY be an indication that be is not properly licensed and is not entitled to perlittiDg privileges in the'
<br />City of Zepbyrbills.

<br />installation bas cmleDced prior to issuance of a pemit and that all work will be perfoIJed to Jeet standards of all laws
<br />regulating construction, City codes, loning regulations, and land developaent regulations in the jurisdiction. I also
<br />certify that I understand that the regulations of other governaental agencies aay apply to the intended work, and that it is

<br />A Perlit issued shall be construed to be a license to proceed witb the wort and not as authority to violate, cancel alter, or
<br />set aside any provisions of tbe technical codes, nor sball issuance of a perlit prevent the Building Official froa thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Every pertit issued shall beCOl8 invalid
<br />unless the work authorized by sueb perl it is COllellced witbin sillOntbs of issuance, or if work autboriled by the perlit is
<br />suspended or abandoned for a periOd of sillOntbs after the tile tbe work is COIIeJlced. One 90 day utension of tile, lilY be
<br />allowed for the perlit witb fee ebarge of $15.00. Tbe 81tension sball be requested in writing to the Building Official. An
<br />approved inspection lUSt be logged during eaeb sillOntb period, or the project will be considered abandoned.
