<br />cited for a lisdeteanor violation under state law. If the owner or intended contractor are uncertain as to wbat licensing
<br />requireJents lay apply for the intended wort, they are advised to contact the City of Zephyrhills Building DepactJent, (813)

<br />"Contractor Sections. of this application for wbich they will be responsible. If you, as the owner sign as the contractor,
<br />you are indicating that you, rather than the contractor, are responsible for the wort. If the contractor wisbes you to sign
<br />as contractor that JaY be an indication that be is not properly licensed and is not entitled to pelllitt~g privileges in the
<br />City of Zepbyrhills.

<br />I certify that all the inforaation in this application is accurate and that all wort will be done in cOJpliance with all
<br />applicable laws regulating construction, loning, and land developlent.

<br />Application is hereby Jade to obtain a perlit to do wort and installation as indicated. I certify that no wort or
<br />installation has cOlleDced prior to issuance of a pelllit and that all wort will be perf oIled to leet standards of all laws
<br />regulating construction, City codes, loning regulations, and land developtent regulations in the jurisdiction. I also
<br />certify that I understand that the regulations of other governtental agencies lilY apply to the intended wort, and that it is

<br />A pertit issued sball be construed to be a license to proceed with the wort and not as authority to violate, cancel alter, or
<br />set aside any provisions of the technical codes, nor shall issuance of a per.it prevent the Building Official fr~ thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Every perait issued shall becoae invalid
<br />unless the wort authorized by such pelllit is cOllellced within six IOnths of issuance, or if wort authorized by the pelllit is
<br />suspended or abandoned for a period of six IOlltbs after the tile the work is coaenced. One 90 day extension of tile, lily be
<br />allowed for the pelllit with fee charge of '15.00. Ibe extension sball be requested in writing to the Building Official. An
<br />approved inspection lust be logged during eacb sixlOnth period, or the project will be considered abandoned.
