<br />requireJents lay apply for tbe intended work, they are advised to contact the City of Zephyrbills Building Departlent, (813)

<br />FurtherlOre, if the owner bas hired a contractor or contractors, be is advised to have the contractor(s) sign portions of the
<br />.Contractor Sections" of this application for wbich tbey will be responsible. If you, as the owner sign as the contractor,
<br />you are indicating tbat you, rather than the contractor, are responsible for the work. If the contractor wisbes you to sign
<br />as contractor that JaY be an indication that be is not properly licensed and is not entitled to pe~itting privileges in the
<br />City of Zephyrbills.

<br />installation bas COIIenced prior to issuance of a perlit and that all work will be perforaed to leet standards of all laws
<br />regulating construction, City codes, zoning regulations, and land developaent regulations in the jurisdiction. I also
<br />certify that I understand tbat the regulations of other goveIDIental agenCies lay apply to tbe intended work, and tbat it is

<br />Florida prior to perlit issuance.

<br />A perlit issued shall be construed to be a license to proceed with the Ifork and not as authority to violate, cancel alter, or
<br />set aside any provisions of the technical codes, nor shall issuance of a per.it prevent the Building Official frOJ thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Every perJit issued sball becOle invalid
<br />unless tbe work authorized by such perlit is cOll8nced witbin six IOnths of issuance, or if Ifork autborized by the per.it is
<br />suspended or abandoned for a period of six IOntbs after tbe tile the work is cOllenced. One 90 day extension of tOe, laY be
<br />allowed for the per~jt with fee charge of $15.00. Tbe extension sball be requested in Ifriting to the Building Official. An
<br />approved inspection lust be logged during eacb six IOnth period, or tbe project will be considered abandoned.
