<br />state and local regulations. If tbe contractor is not licensed as required by law, both the owner and contractor lilY be
<br />ci ted for a lisdl!leanor violation under state law. If the owner or intended contractor are uncertain as to wbat licensing
<br />requireleDts laY apply for the intended work, they are advised to contact the City of Zepbyrbills Building Departlent, (813)

<br />FurtherlOre, if the owner has bired a contractor or contractors, be is advised to have the contractor(s) sign portions of the
<br />-Contractor Sections. of this application for wbich they will be responsible. If you, as the owner sign as the contractor,
<br />you are indicating that you, rather tban the contractor, are responsible for the work. If the contractor wisbes you to sign
<br />as contractor that laY be an indication that be is not properly licensed and is not entitled to pellitting privileges in the
<br />City of Zephyrbills.

<br />Application is hereby lade to obtain a pellit to do work and installation as indicated. I certify that no work or
<br />installation bas cOlleDced prior to issuance of a pellit and that all work will be perfoIJed to leet standards of all laws
<br />regulating construction, City codes, zoning regulations, and land developleDt regulations in the jurisdiction. I also

<br />Florida prior to perlit, issuance.

<br />A perlit issued shall be construed to be a license to proceed with the work and not as authority to violate, cancel alter, or
<br />set aside any provisions of the technical codes, nor shall is~uance of a perlit prevent the Building Official fIOl thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Every petlit issued sball beCOJe invalid
<br />unless the work authorized by such pellit is cOlleDced within six IOnths of issuance, or if work authorized by the petlit is
<br />suspended or abandoned for a period of six IOntbs after the tile the work is c~ced. One 90 day extension of tille, IIBY be
<br />allowed for tbe pellit with fee charge of $IS.OO.The extension shall be requested in writing to the Building Official. An
<br />approved inspection lUst be logged during each six IOnth period, or the project will be considered abandoned.
