<br />cited for a lisdeteanor violation under state law. If the owner or intended contractor are uncertain as to wbat licensing
<br />requirelents lay apply for tbe intended work, they are ad;iaed to contacl lhe City of Zepbyrbills Building DepartJent, (813)

<br />FurtherlOre, if the owner bas bired a contractor or contractors, be is advised to have tbe contractor(s) sign portions of the
<br />"Contractor Sections" of this application for wbicb tbey will be responsible. If you, as the owner sign as the contractor,
<br />you are indicating that you, ratber than tbe contractor, are responsible for tbe work. If tbe contractor wishes you to sign
<br />as contractor that lay be an indication that be is not properly licensed and is not entitled to perlitting privileges in the
<br />City of Zepbyrhills.

<br />applicable laws regulating construction, loning, and land developlent.

<br />Application is hereby lade to obtain a perlit to do work and installation as indicated. I certify that no work or
<br />installation bas cOlleDced prior to issuance of a perait and that all work will be perforaed to aeet standards of all laws
<br />regulating construction, City codes, loning regulations, and land developaent regulations in the jurisdiction. I also
<br />certify tbat I understand that the regulations of otber goverDIeRtal agencies lay apply to tbe'intended work, and that it is
<br />IY responsibility to identify what actions I lust take to be in cOlpliance. Sucb agencies include but are not lilited to:

<br />Florida prior to perlit. issuance.

<br />A perlit issued sball be construed to be a license to proceed with tbe work and not as authority to violate, cancel alter, or
<br />set aside any provisions of tbe technical codes, nor shall issuance of a perlit prevent tbe Building Official frOl thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Every perait issued sball becOle invalid
<br />unless the work authorized by such perlit is cOllenced within six IOnths of issuance, or if work authoriled by the perait is
<br />suspended or abandoned for a period of sillOnths after tbe tiae the work is cOllenced. One 90 day extension of tiae, laY be
<br />allowed for the perait with fee charge of $15.00. Tbe extension sball be requested in writing to the Building Official. An
<br />approved inspection lUst be logged during each six IOnth period, or the project will be considered abandoned.
