<br />state and local regulations. If the contractor is not licensed as required by law, both the owner and contractor lilY be
<br />cited for a lisdeleanor violation under state law. If the owner or intended contractor are uncertain as to wbat licensing
<br />requirelents lilY apply for the intended work, they are advised to CQIltact the City of Zepbyrhills Building Departlent, (813)
<br />788-6611.

<br />'Contractor Sections' of this application for which they will be responsible. If JOU, as the owner sign as the contractor,
<br />you are indicating that JOU, rather than the contractor, are responsible fot the work. . 'If' the Coiltractor wishes you to sign
<br />as contractor that lilY be an indication that he is not properly licensed and is not entitled to peraitting privileges in the
<br />City of Zephyrhills.

<br />suspended or abandoned for a period of sillODtbs after the tHe the work is co.encecl. one 90 day atension of tHe, lilY be
<br />allowed for the perlit with fee charge of $15.00. !he atension sba11 be requested in writing to the Building Official. An
<br />approved iDBpection lUSt be :logged during each sillODth period, or the project will be considered abandoned.
