<br />cited for a lisdeteanor violation under state law, If the owner or intended contractor are uncertain as to wbat licensing
<br />requirBlents laY apply for the intended work, they are advised to contact the City of Zephyrbills Building Departlent, (813)

<br />you are indicating that you, rather than the contractor, are responsible for tbe work. If tbe contractor wishes you to sign
<br />as contractor that lay be an indication that he is not properly licensed and is not entitled to perlitting privileges in the
<br />City of Zephyrbills.

<br />installation bas COIIeDced prior to issuance of a perlit and that all worl will be perfocted to teet standards of all laws
<br />regulating construction, City codes, loning regulations, and land developteDt regulations in the jurisdiction. I also
<br />certify that I understand that the regulations of other governtental agencies laY apply to the intended work, and that it is

<br />A perlit issued shall be construed to be a license to proceed witb tbe work and not as authority to violate, cancel alter, or
<br />set aside any provisions of the technical codes, nor sball issuance of a perlit prevent the Building Official froa thereafter
<br />requiring a correction of errors in plans, construction, or violations of any code. Every perait issued sball beCOle invalid
<br />unless tbe If art autborhed by sucb perlit is co.nced within slllOnths of issuance, or if wort author lied by the perlit is
<br />suspended or abandoned for a period of sll IOntha after the tile the wort is cOllenced. One 90 day eatension of tile, tay be
<br />allowed for the perait with fee cbarge of $15.00. The eatension sball be requested in writing to the Building Official. An
<br />approved inspection lust be logged during each sillOntb period, or the project will be considered abandoned.
