Morris, President <br />REGAL LANES, INC. <br />4847 Armenia Avenue <br />Tampa, FL 33603 <br /> <br />Jobsite: <br /> <br />Unity Lanes <br />Zephyrhills, FL <br /> <br />Undersigned Owner represents that he/she is the Owner of the property and has full <br />authority to negotiate and execute contracts concerning the property. <br /> <br />We propose to furnish all materials and perform all labor necessary to complete the <br />following: <br /> <br />Remodel and redesign existing interior structure to allow for two new offices, ne pro <br />shop, additional storage place and provide better traffic flow in concourse area. This will <br />include <br /> <br />. Demolition of existing walls per plan <br />. Construct pony wall at West end, close the door opening and window at pool <br />room. <br />. Construct wall back of counter with display case. <br />. Divide area into 2 offices 11.8 x 11.8 per plan <br />. Construct two storage areas secured with plywood celings at back side of coolers <br />. Construct pro shop at East end with drill room, manager's office and storage room <br />. Construct pony wall and locker room walls at East end. <br />. Replace both bath doors, and build bench in locker room. <br />. New bar tops and control center tops <br /> <br />Upon receipt of deposit, undersigned Contractor shall immediately schedule <br />construction. Work shall progress steadily, and shall be completed in a timely fashion. <br />All of the above work is to be completed in a substantial workmanlike manner according <br />to standard practices and requirements of the governing authority (city, state, county). <br />Contractor shall obtain and be responsible for all permits and inspections necessary. In <br />the event, due to implementation of new requirements by the governing authorities, <br />additional fees are assessed, Owner shall be responsible for payment thereof. <br /> <br />Contractor is fully licensed in the State of Florida as a Class A General Contractor, <br />License #CGC02 7 816, bonded and insured for liability. Contractor shall take all <br />prudent precautions to prevent theft, vandalism or other loss, but is not financially <br />responsible for same. Should Owner require insurance against such loss during <br />construction, he is responsible for payment of same, Contractor to furnish "no lien" <br />affidavi t upon final payment and completion of work, provided Owner has paid all <br />monies due in full. <br />
