Final Site Plan - Radiation Oncology - Alex Panik <br /> <br />A revised site plan was submitted addressing the following issues: <br /> <br />· A note has been added to the revised plans stating that the owrier agrees to extend <br />a 24' wide paved traffic aisle when the property to the north is developed~ <br />· A five foot wide sidewalk has been added on the east side of property~ <br />· One fire hydrant has been added on the northeast comer of Green Slope Drive and <br />Medical Arts Cowt as requ,ested by the Fire Department~ <br />· $150.00 Site Plan Review fee was received <br /> <br />The owner will coordinate with the Public Works Department conceriling dumpster pad / <br />enclosure requirements. The fmal site plan was approved. . <br /> <br />2. Revised Site Plan - Copeland Steel <br /> <br />A revised site plan was submitted showing an existing unpermitted modular office <br />building and some proposed crane rails. Mike Johnson was present representing Towson <br />& Rogers. The setbacks and parking requirements are met for the improvements. The <br />Site Plan was approved with the following conditions: <br /> <br />· SWFWMD permit / exemption needs to be provided to the City~ <br />· A letter need to be provided from the County Health Department for any septic <br />requirements for the modular office building; <br />· Building permits need to be applied for prior to October 24, 1997. Permitting not <br />completed prior to this date will result in fmes~ and <br />· $150.00 Site Plan review fee shall need to be submitted to the City prior to <br />October 7, 1997. <br /> <br />3. Dulcie Benson - Status <br /> <br />Staff reviewed a preliminary site plan on April 9, 1997. A 90 day deadline was given to <br />complete setup, renovation and site work. No work has apparently been done. Staffwill <br />contact Fred Deuel to see if the final requirements have been completed.
