UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES <br />If the owner has hirect a contractor OJ:: contractors to und.ertake WOJ;:k, they may be req:uired <br />to be licensed in accordance with state and local regulation~. If the contractor is not <br />liceoseci aa required by law, both the owner and contractor may be cited for a misdemeanor <br />violation unQer state law. It the owner or intended contractor are uncertain as to what <br />licensing requirements may apply for the intended work, they are advised to contact the <br />city of Zephyrhills Building Department, 813-766-6611. <br />Furthermore, if the owner has hired a contractor O~ contractors, he is advised to have the <br />contracto~(s) sign portions ot the "Contractor Sections>> of this application for which they <br />will be re6ponsible. It you, aa the owr~er signs as the contractor, you are indicating that <br />you, rather than the contractor, are responsible for the work. If the contractor wishes <br />you to si~n as contractor that may be an indication that he is not properly licensed and is <br />not entitled to permitting privileges in the city of Zephyrhills. <br />C. If the applicant is someone other that the "owner", I cerify that I <br />have obtained a copy ot the above descr~bed document and promise in good faith to deliver <br />it to the ~ownerH prior to commencement. <br />E. CONTRACTOR'S/OWNER'S AFFIDAVIT <br />1 certify that all the information .in this applicati.on is accurate and that all work will <br />be done in c~npli~nce with all app11cable laws regulating construction, zoning, and land <br />development. <br />Application is he~eby made to obtain a permit to do work and installation as indicated. I <br />certify that no work or installation has commenced prior to issuance of a pertnit and that <br />allwo.rk will be pe:rfo:cmed to meet standards of all laws regulati.ng construction, City <br />codes, zoning regulations, and land development regulations in the ju~isdiction. Every pepmit <br />issued shall become invalid unless the work authorized by such pe:,.:ndt is cowClenced within <br />six months ot issuance, or if work authorized by the pe~~t is euapended or abandoned for a <br />period of six month.:!l after the time the wox:k is commenced. One 90 day extension of time <br />may be allowed for the permit with fee oharge of $15.00. The extension shall be requested <br />j,n wr.1t.ing to the Building Offici-al.
