<br />violation under state laH. It the owner or intended contractor are uncertain as to Ylh~t
<br />ljcensing requirements may apply for the intended work, they are advised to contact the
<br />,ci ty of Zephyrhi1ls l3uilding Department, 813-768-6611.
<br />Furtbennore, if the owner has hired a contractor or oontractors, he is advised to have the
<br />contractor (B) sign portions of the "Contractor Sections" of this appli.cation for Hhiah they
<br />will be responsible. If you, as the owner s.igns 'IS the contractor, YQl,\ are indioating that
<br />you, rather than the contractor, are responsible for the work. If the contractor wishes
<br />you to sign as contractor that may be an indication that he is not properly liaellsed and is
<br />not entitled to permitting privil~ges in the City of Zephyrhills.

<br />lien Law - Homeowner's Protection Guide" prepared by the Florida Department of Agriculture
<br />and Consumer Affairs. If the applicant is someone other that the "owner/', I cerify that I
<br />have obtained a copy of the above described document and promise in good faith to deliver
<br />.i t to the "owner'l prior to corrunencement.

<br />be done in compliance with all applicable laws regulating construction, zoning, and land
<br />development.
<br />Applioation is hereby made to obtaln a permit to do work and installation as indioated. 1
<br />certi fy that no \wrk or installati_on has couunenced prior to issuance of a permi t and that
<br />all work will be performed to meet standards of all laws regulating construotion, City
<br />codes, zoning regulations, and land development regulations in the jllrisdictlon. I also
<br />certify that I understand that the regulations of other governmental agencies lOay apply to
<br />the intended work, and that it is my responsibility to identify what actions I must take to
<br />be in compliance. Such agencies include but are not limited to: *Department of
<br />Environmental Regulation-Cypress Bayheads, Wetland Areas and Environmentally Benettiva
<br />Lands, Water/Wastewater Treatment

<br />I also certify that, if fill material is to be used in Flood Zone qA" or "A/etc.", lt is
<br />understood that a drainage plan addressing a "compensating volume"'will be submitted Y/hich
<br />is prepared by a professional engineer registered in the State of Florida prior to permit
<br />jssuance.
<br />A permit issued shall be construed to tie a license to proceed with the work and not as
<br />authority to violate, cancel, alter, or set aside any provisions of the technical codes,
<br />nor shall issuance of a permlt prevent the Building Official from thereafter requi.ring a
<br />correction of errors in plans, oon~truction, or violations of any oode. Every pe~~it
<br />iSIIUed shall beoome invalid unlel9l:j the work authori:z.ed by such permit is cOllunenced ~i1ithin
<br />six montbs of issuance, or if work authorized by the permi. t is suspended Or abandor~ed for a
<br />period of alx months after the time the work is conunenced. One 90 day extension of time
<br />may be alloHed for the permit with fee charge of $15.00. The extension shall be teques~ed
<br />in wri ting to the Building Official. An approved inspection must be logged during each I:d H
<br />month period, or the proj ect vilill be considered abandoned.
