<br />A. NOTICE OF DEED RESTRICTIONS
<br />The undersigned understands that this permit nlay be subject to. ~deed restrictians" which
<br />may be mare restrictive than City regulatians. The undersigned assumes responsibility far
<br />campliance with any applicable deed restrictions.
<br />B. UNLICENSED CONTRACTORS AND CONTRACTOR RESPONSIBILITIES
<br />If the awner has hired a cantractar ar cantractars to undertake wark, they may be required
<br />to be licensed in accordance with state and local regulations. If the contractor is not
<br />licensed as required by law, bath the owner and cantractor may be cited for a misdelneanor
<br />vialation under state law. If the owner or intended contractor are uncertain as to. what
<br />licensing requirements may apply far the intended wark, they are advised to. cantact the
<br />City af Zephyrhills Building Department, 813-788-6611.
<br />Furthermare, if the owner has hired a cantractar or cantractars, he is advised to. have the
<br />cantractar(s) sign partians af the ~Contractor Sections" af this applicatian far which they
<br />will be respansible. If you, as the owner signs as the cantractar, yau are indicating that
<br />yau, rather than the cantractar, are respansible far the wark. If the cantractar wishes
<br />yau to. sign as cantractar that may be an indication that he is nat praperly licensed and is
<br />not entitled to permitting privileges in the City of Zephyrhills.

<br />I certify that all the infarmation in this applicatian is accurate and that all wark will
<br />be done in campliance with all applicable laws regulating canstructian, zaning, and land
<br />develapment.
<br />Applicatian is hereby made to. abtain a permit to. do. work and installatian as indicated. I
<br />certify that no. wark ar installatian has cammenced priar, to. issuance af a permit and that
<br />all wark will be perfarmed to. meet standards af all laws regulating canstructian, City
<br />eades, zoning regulations, and land develapment regulations in the jurisdiction. I also
<br />certify that I understand that the regulations af ather gavernmental agencies may apply to.
<br />the intended work, and that it is my respansibility to. identify what actions I Inust take to.
<br />be in campliance. Such agencies include but are nat limited to.: *Department af
<br />Enviranmental Regulatian-Cypress Bayheads, Wetland Areas and Enviranmentally Sensitive
<br />Lands, Water/Wastewater Treatment

<br />understood that a drainage plan addressing a ~campensating volume" will be submitted which
<br />is prepared by a prafessional engineer registered in the State af Florida prior to permit
<br />issuance.
<br />A permit issued shall be const~ued to. be a license to proceed with the work and nat as
<br />autharity to violate, cancel, alter, or set aside any provisions of the technical codes,
<br />nar shall issuance af a permit prevent the Building Official from thereafter requiring a
<br />correction of errars in plans, construction, or violations af any cade. Every permit
<br />issued shall become invalid unl~ss the work autharized by such permit is cammenced within
<br />six months af issuance, ar if wqrk authorized by the permit is suspended or abandoned for a
<br />peri ad of six manths after the t~me the wark is cammenced. One 90 day extensian of time
<br />may be allowed for the permit with fee charge of $15.00. The extension shall be requested
<br />in writing to. the Building OfficIal. An appraved inspectian must be lagged during each six
<br />manth periad, ar the praject will be cansidered abandoned. '"
