If the applicant is someone other that the "owner", I cerify that I <br />have obtained a copy of the above described document and promise in good faith to dellver <br />it to the "ovmel:" prior to COllunencement. <br />E. CONTRACTOR'S/OWNER'S AFFIDAVIT <br />I certify that all the information in this application is accurate and that all Hark will <br />be done in compliance I.lith all applicable laws regulating construction, zoning, and land <br />devt::lopment. <br />Application is hereby made to obtain a permit to do Hork and installation as indicated. I <br />certi fy that no work or installation has cOllunenced prior to issuance of a penni t and that <br />all Hork will be performed to meet standards of all laws regulating constrllction, City <br />codes, zoning regulations, and land development regulations in the jurisdiction. 1 also <br />certify that I understand that the regulations of other governmental agencies may apply to <br />the intended vwrk, and that it is my responsibility to identify what actionl:> I must t.ake to <br />be in compliance. Every permit <br />il:>sued shall become invalid unless the work authorized by such permit is couunenced within <br />six montlls of issuance, or if work authorized by the permit is suspended or abandoned for a <br />period of six monthl::l after the time the Hark is conuuenced. One 90 day extension of time <br />may be alloHed for the permit with fee charge of $15.00. 'rhe extension shall be requested <br />in viri ting to the Building Official.
