<br />cited for a lisd_anor violation under state law. If the mmer or Intended contractor are uncertain as to wbat licensing
<br />requireaents laY apply for the intended work, they are advised to contact the City of Zepbyrbills Building Departlent, (813)

<br />"Contractor Sections- of this application for wbicb they will be responsible. If you, as the OIRler sign as the contractor,
<br />you are indicating that you, rather than the contractor, are responsible for the work. If the contractor wisbell you to sign
<br />as contractor that lilY be an indication that be is not properly licensed and is not entitled to peraitting privileges in the
<br />City of Zepbyrbills.

<br />requiring a correction of errors in plans, construction, or violations of any code. Every perlit issued sball becOle invalid
<br />unless the work authoriled by sucb perlit is cOBellced lithin sillODtbs of issuance, or if work autboriled by the peIlit is
<br />suspended or abandoned for a period of sillOntbs after the tile the work is coaenced. One 90 day utension of tile, lilY be
<br />allowed for the perait with fee charge of $15.00. Tbe utension sball be requested in lriting to the Building Official. An
<br />approved inspection lUSt be logged during eacb sil IOnth period, or the project will be considered abandoned.
