The permit authorizes the Permittee to <br />proceed with the construction of a surface water management system in accordance with the information <br />outlined herein and shown by the application, approved drawings, plans, specifications, and other <br />documents, attached hereto and kept on file at the Southwest Florida Water Management District <br />(District). Unless otherwise stated by permit specific condition, permit issuance constitutes certification of <br />compliance with state water quality standards under Section 401 of the Clean Water Act, 33 U.S.C. 1341. <br />All construction, operation and maintenance of the surface water management system authorized by this <br />permit shall occur in compliance with Florida Statutes and Administrative Code and the conditions of this <br />permit. <br /> <br />PROJECT NAME: <br />GRANTED TO: <br /> <br />Ramappa Medical - Zephyrhills <br /> <br />East Pasco Medical Arts Inc. <br />12136 Cobblestone Drive <br />Bayonet Point, FL 34667 <br /> <br />ABSTRACT: This permit is for a modification of a system previously permitted by 8WFWMD, entitled <br />Merchants Square- Townview Medical Arts (Management Storage Surface Water [MSSW] No. <br />40009944.000). The permit authorizes construction of an office and associated parking within a 1.21-acre <br />outparcel, which was included in the master surface water system. This permit modification will replace <br />Environmental Resource Permit (ERP) No. 44009944.007, Townview Medical Arts Building - Phase II, <br />issued on July 26, 2oo1.The project site is located off US Highway 301, one mile north of Zephyrhills, <br />Pasco County. Adjacent properties include previous permits associated with the Merchants Square. <br /> <br />The proposed surface water management system conveys runoff to the master drainage system for the <br />project. Water quality and quantity requirements are met in the master system. There are no wetlands <br />within the project area. <br /> <br />The Engineer of Record, Richard Matassa, Florida P.E. No. 51431, has submitted design calculation <br />information and construction drawings which indicate no adverse off-site water quality or quantity impacts <br />will result from construction of this project. The on-site surface water management system will be <br />maintained and operated by the Permittee. The off-site surface water management system will be <br />maintained and operated by the Townview Medical Arts Center Owner Association.
