Your eventual Certification of Construction <br />Completion in accordance with the plans is necessary before we will issue the Operating Permit. <br /> <br />Four copies of DOH Form 916 (Application of a Swimming Pool Operating Permit/Authorization) <br />are enclosed, Three executed copies of this form need to be returned to us after the pool or spa <br />is completed and accepted. The pool or spa may not be used for its intended purposes <br />until we have issued final approval. <br /> <br />This approval is based solely on the representations and data furnished to this <br />department. There may be county, municipal, or other local regulations or restrictions <br />requiring compliance before construction of the above-mentioned pool or spa is <br />initiated; therefore, I recommend that you consult with appropriate local agencies before <br />starting construction.
