The undersigned states and affirms that he or she is desirous of constructing, renovating, adding to or reroofing his or her own domici~e~, that he or she actua~~y occupies, or wi~~ occupy by 'said domici~e, and same is not for rent, ~ease or sa~e. That he or she sha~~ comp~y with the fo~~owing conditions.: 1. That the owner and he or she a~one sb.aJ.~ act as the bui~derfor a~~ phases of" construction. That the owner wi~~ comp~y with a.J.~ provisions of the City of Zephyrhi~~s ordinances and codes pertinent to 'the bui~ding. That :in the event various phases of construction engage on1y proper~y ~icensed subcontractors and such work., That in the event the Bui~ding I:nspector sha~~ require corrections to be made, the owner wi~~ assume fuJ.~respons:ibi~ity to insure the.y are made, and upon comp~etion wi.~i cal~ for a reinspection before proceeding with the bui~ding. That the oWner shal~ assume full respons:ibi~ity for the construction and wi~~ not expect supervision of his Work from the City of Zephyrhi~~s Bui~ding Department. That prior to final inspection any additiona~ fees, fnc~uding re:inspection fees, must be paid in fuJ.~. A written request from this office shall ,constitute' an officia2 notice, to pay additional fees. That the owner shal~ comply with all City, State and Federa.J. laws' in regard to social security, workman's compensation, lien ~aws, etc., where applicable. That the owner shall comply with a~l the safety codes issued by the Florida Industria~ Commission. ' State law requires construction to be done by ~icensed contractors. You have app~ied for a permit under an exemption to that ~aw. The exemption allows you, as the own,er of your property, to act as your own contractor with certain restrictions even though you do not have a ~icense. You must provide direct ons:ite supervision of the construction yourse~f. You mci.y bui~d or imprc:ve ,a one-family or two-family residence or a far.m outbui~ding. You may also build or improve a commercia~ bui~ding, provided your costs do not exceed $25,000. The bu:i.~ding or residence must be for your own use or occupancy. I:t may not be bu:i.lt or, s;msta.ntial~y improved 'for sa~e or ~ease. You may not hire an unJ.icensed person to act as your contractor or to supervise peop~e working on your building. I:t is your respons:ibi~i ty to make sure that people emp~oyed by you have licenses required by state law and by county or municipal licens:i.ng ordinances. You may not delegate the responsibi~ity for supervising work to a licensed contractor who is not ~ice:a.sed to perfoJ:m the work being done. Any person working on your building who is not licensed must work under your direct supervision and must be employed by you, which means that you must deduct F.I:.C.A. and w:Lth.b.olding tax and provide workers' compensation for that emp~oyee, a~~ as prescribed by 1aw.
