DISCLOSURE STATEMENT FOR OWNER CITY OF ZEP:EYREILLS B'C'J:LDING DEPARTMENT I, have read and ful.~y understand and agree to the prov~s~ons of thi.s ~nstrument. The unders~~ed states and aff~r.ms that he or she is des~rous of construct~ng, renovat~ng, adding to or reroof~ng his or her own domicile;, that he or she actually occup~es, or wil~ occupy by ,'sa~d domic~~e, and same ~s not for rent, ~ease or sale. That he or she shall comply with the :Eollow~ng condit~ons.: L That the owner and he or she alone sha.1.1 act as the builder .for all phases of. construct~on. That the owner w~ll comply with all prov~s~ons of the C~ty of Zephyrhills ordinances and codes pert~nent to "the building. That ~n the event var~ous phases of construct~on are subcontracted, he w~ll engage only properly l~censed subcontractors and w~ll personally superv~se such work.. That in the event the Building Inspector sha.1.1 require correct~ons to be made, the owner w~ll assume full responsib~l~ ty to ~nsure they are made, . and upon complet~on wili call for a reinspect~on before proceeding with the building. That the oWner shall assume fuJ..l responsib~~~ty for the construct~on and w~ll not expect superv~s~on of his Work from the C~ ty of Zephyrhills Building Department. That pr~or to f~nal inspect~on any addit~onal fees, ~ncluding re~nspect~on fees, must be pa~d ~n full. A wr~tten request from this off~ce shall .const~tute. an off~c~a~ not~ce"to pay addit~onal fees. That the owner shall comply with all C~ty, State and Federal laws. ~n regard to soc~al secur~ ty, workman' s compensat~on, l~en laws, etc., where appl~cable. That the owner shall comply with all the safety codes issued by the Flor~da J:ndustr~al Commission. State law requires construct~on to be done by l~censed contractors. You have appl~ed for a permit under an exempt~on to that law. The exemption allows you, as the own,er of your property, to act as your own contractor w~th certain. restr~ct~ons even though you do not have a l~cense. You must prov~de direct onsite supervision of the construction yourself. You may build or improve a one-fam.:ily or two-fam.:ily residence or a fa:rm outbuilding. You may also 'build or improve a commercial building, provided your costs do not exceed $25,000. The building or residence must be for your own use or occupancy. It may not be built or, s~stant~ally improved for sale or lease. If you s~l :o~ lease a building you. have built or . substantially improved yourse1f with.J.n 1 year after the construct~on is complete, the law w~ll presUmeth&t you bu~lt or substant~ally improved ~f for sale or lease, which ~s a violation of this exemption. You may not hire an unlicens'ed person to act as your contractor or to supervise people worlcing on your building. It is your responsib~li ty to make sure that people employed by you have l~censes required by state law and by county or municipal l~cens~ng ordinances. You may not delegate the responsibility for supervis~ng work to a l~censed contractor who is not l~censed to perform the work being done. Any person working on your building who is not licensed must work under your direct superv~sion and must be employed by you, wh.ich means that you must deduct F.I.C.A. and wi.tb.holding tax and provide workers' compensat~on for that employee, all as prescribed by iaw.
