<br />ATTACHMENT <br /> <br />Bid/Contract Documents <br />(Plans, timelines, cost breakdowns and specifications included in bid packages to become part of the <br />Agreement) <br /> <br />(b) Commercial General Liability Insurance. Commercial General Liability Insurance <br />written on an "occurrence" basis with a combined single limit of at least $1,000,000 per occurrence, and <br />a general aggregate of $2,000,000, in forms providing coverage not less than the standard commercial <br />general liability policy including hazards of operation, broad form property damage liability coverage, <br />products/completed operations coverage, independent contractor coverage and broad form contractual <br />coverage for liability assumed under the Agreement, to the extent insurable under the policy. Coverage <br />shall include liability arising out of acts of agents or Subcontractors of Contractor. <br /> <br />(c) Automobile Liability Insurance. Coverage for all motor vehicles operated by or <br />for Contractor, including protection for automobiles and trucks used by Contractor either on or away <br />from the sites at which Work is being performed, with a combined single limit of at least $500,000 per <br />occurrence for bodily injury and property damage. The policy shall include coverage for all hired, <br />owned and non-owned vehicles. <br /> <br />(d) "All Risk" Property Insurance. If in conjunction with the Work to be performed by <br />Contractor under the Agreement, Contractor stores, warehouses, transports or otherwise has in its <br />possession at the Project site, or at a location other than the Project site, any equipment, materials or any <br />other personal property owned or leased by Owner, then Contractor shall (1) be fully responsible for said <br />equipment, material or other personal property while in Contractor's possession, and (2) purchase and <br />maintain "All Risk" property insurance policy on all of Contractor's buildings or facilities including, <br />without limitation, coverage for sprinkler leakage, boiler and machinery (comprehensive form), transit, <br />theft, flood, and earthquake (at California facilities only). The policy shall be endorsed to also cover <br />damage to or loss of personal property owned or leased by Owner, trade fixtures, inventory fixtures or <br />other equipment located in, on or about Contractor's premises or while in transit. Such insurance is to <br />be written on a replacement cost basis without deduction for depreciation, in an amount equal to 100% <br />of the full replacement value in the aggregate of all the items referred to in this Section. The policy shall <br />include a loss payee endorsement in favor of Owner to the extent of its insurable interest in the covered <br />property. <br /> <br />(e) Commercial Umbrella/Follow Form Excess Policy. Excess liability policy with <br />limits of not less than $1,000,000 per occurrence in excess of the primary underlying policy limits. The <br />policy must provide coverage at least as broad as the underlying policies. <br /> <br />(f) Professional (Errors and Omissions) and Pollution Legal Liability Insurance (or <br />Contractor's Pollution Liability Insurance). Coverage with minimum limits of $1,000,000 per incident <br />and $2,000,000 annual aggregate covering both on-site and off-site claims for bodily injury, property <br />damage and/or clean up expenses resulting from pollution conditions or any environmental impairment <br />occurring as a result of Services performed by the Contractor or its subcontractors under the Agreement. <br />The policy shall include coverage for both sudden and gradual pollution conditions, and, where allowed <br /> <br />128770 <br />Form <br />4400158 <br />5/04 <br /> <br />GC <br /> <br />
