NOTE TO FILE: Crossroads Pentecostal Church
5208 8th Street
Zephyrhills, FL 33542

9/17/07
Julio Pantoja called the office to advise us that Milbar Construction and the church had signed an
agreement to install a galvanized metal roof on the church. After checking the minutes from the
August 14th Historical Preservation Meeting, the motion was to deny the shingle roof but did not
allow for approval if they changed their minds and installed the metal roof.

Chairman Geiger agreed to sign the Certificate of Appropriateness if the Board agreed by a
telephone vote. I contacted Jodi Wilkeson and she requested that before she voted in favor she
wanted Bill Back to review the Change Order No. 1 that was faxed by Milbar Construction to the
City. I delivered a copy to Bill this evening with a request that he respond tomorrow morning.

9/18/07
Bill Back left a message this morning stating that the C.O. 1 looked fme and he didn't have any
problems with it. The vote was as follows:

Bill Back - yes
John Geiger - yes
Jodi Wilkeson - yes
Jason Aiken - yes
Tracy Sullivan - yes
