<br />maintain an atmosphere lethal to Drywood Termi tes during fumigation,
<br />· to perform services and apply necessary chemicals In a workmanlike manner with methods and materials
<br />established by official research as being effective against the above mentioned Insect pests,
<br />· to open bulldlng(s) and properly aerate.
<br />
<br />MURRELL PEST CONTROL, INC. employees will exercise care in regard to trees, plants, roofs and all parts of the structure(s).
<br />Due to the use of heavy equipment about the structure(s), the' owner / agent hereby releases MURRELL PEST CONTROL, INC.
<br />and its employees of any and all claims for damages sustained during performance of fumigation to:

<br />date, This service guarantee shall provide forwhateverretreatments, including re-fumigation, deemed necessary by MURRELL PEST CONTROL,
<br />INC, to control any active infestation ofthe above mentioned insect pests at no extra cost.

<br />1 7 4 MURRELL PEST CONTROL, INC. assumes no responSibility or liability with regards from the owner ofthe annual sum of
<br />$ . U 0, payable on or before each anniversary date of this contract for 5 years. Reinspection of property will be made upon request.

<br />, Owner warrants full cooperation with MURRELL PEST CONTROL, INC, to maintain treated areas free from any factor or conditions
<br />contribu~ing to the infestation of the above mentioned insect pests and agrees that any additions or alterations, after fumigation, are not covered

<br />Should this property be sold, this contract shall pass with title to said property, provided all provisions ofthis contract have been complied
<br />with by the owner and that MURRELL PEST CONTROL, INC, is notified of such changes of ownership.
<br />NOTE: This contract does not Include treatment, service or Q.uarantee for Subterranean tennites as they require an
<br />entirely different type of treatment

<br />You the buyer, have the right to cancel this contract within three (3) business days of the date of this transaction, provided work has not
<br />been started,
<br />
<br />In the event this account is referred to an attorney for collection, customer agrees to pay all costs of collection, including court costs, and reasonable
<br />attomey fees, interest at 1 1/2% per month (18%) charged on all invoices not paid within 30 (thirty) days.
