I I" .---- <br /> <br />5 Will the customer be available to sign for delivery? ~ D ~ I <br />(If not, the customer must add an authorized person to the account before de Ivery will <br />be available.) <br /> <br />[]X] <br /> <br />D <br /> <br />D <br /> <br />@] <br /> <br />.....r- <br />" f7( <br /> <br />\ f3{ <br /> <br /> <br />A Delivery includes a one time building placement and leveling (up to 6 inches). <br />8 If the customer requires the delivery driver to perform duties that make the delivery longer <br />than 45 minutes, the customer agrees to pay a charge of $40.00 per hour. <br />C The customer may incur an attempted delivery fee if the answers to questions 1-5 above <br />prove inaccurate and the driver is dispatched but cannot deliver the building. <br />D The customer must pay all deposits, fees, delivery costs, and applicable taxes before the <br />scheduling of Lease with option to purchase orders. <br />E The customer must pay a non-refundable deposit (amount to be determined by <br />management) on special built buildings prior to the order being processed for construction. <br />F Cook Portable Warehouses carry a lifetime warranty against termite damage and decay. <br />Therefore, it is not necessary to place them on blocks. However, at the customer's request <br />at the time of placing this order, the driver will block the buildings no more than 1 concrete <br />block high provided the customer has the blocks on hand at the time of delivery. There is a <br />charge of $30.00 for bUildings up to 16' and $40.00 for buildings up to 24'. Buildings <br />longer than 24' must be placed on level ground. <br />G The Lease with option to purchase program has been explained. The customer understand <br />that the term of the agreement is for 36 months and that 55% of each rental payments <br />(excluding taxes, fees and other charges) will be applied towards the purchase price if the <br />option to purchase is exercised. <br />H The customer understands that all rental payments are due on the last day of each month. <br />Also, if the bUilding is delivered between the 1st and the 15th of the month, the first rental <br />payment is due by the last day of the same month.
