<br />Name changes require legal documentation verifying the name change, which must be mailed to DBPR. An ori~inal, a certified
<br />copy, or a duplicate copy of an original or certified copy of a document that shows the legal name cnange WIll be accepted,
<br />unless DBPR has a question about the authenticity of the document.
