<br />contractor is not licensed as i'equired by law, boIh the owner and contractor may be cited for a misdemeanor violation

<br />intended work, they are advised to conIad the Pasco County Building Inspection Division--licen Section at 727~7-
<br />8009. Fur1helnlOfe, if the owner has hired a contractor or COIlIIactua.. he is advised to have the COIdlac.:b(S) sign
<br />portions of the "COIdrac:;to( Block" of this appIic;ation for whlch they wi! be responsiJIe. If YOU. as the owner sign as the
<br />contractor. that may be an indication that he is not property licensed and is not entilIed to permiIIing privIeges in Pasco
<br />County.
<br />TRANSPORTATION IIIPACTIUTIUI1ES IMPACT AND RESOURCE RECOVERY FEES: The U1ldeuilQded understands
<br />that Transportation Impact Fees and Recourse Recovery Fees may apply to the construction of new buildings, change of
<br />use in existing buildings. or expansion of existing buildings, as specified .. Pasco County 0rdInlnle runber 89-07 and
<br />90-07, as amended. The oodenIigned also 1.I1dersIands, that such fees, as may be due, wi! be identiliecI at the tme of
<br />permitting. It is further U1lderstood that TransporlaliOl'l Impact Fees and Resourt:e Recovery Fees must be paid prior to
<br />receiving a "oertifk;ate of occupancy" or final power release. If the project does not involve a oettillcate of occupancy or
<br />final power release, the fees must be paid prior to permit issuance. Furthermore, If Pasco County WaterISewer Impact
<br />fees are due, they must be paid prior to permit issuance in accordance with ~ Pasco County UI~_lC8S.

<br />certiry that I, the applicant, have been provided with a copy of the "Florida Construc:tion Lien Law Ilomeowner's
<br />Protediol. GuIde" prepared by the FlorIda Department of AgrlcuIlure and Consumer AffaIns. If the applicant is someone
<br />other than the "owner". I certify that I have obtained a copy of the above described document and promise .. good faith to
<br />deliver it to the "owner" prior to c:onmencement.

<br />If the fill material is to be used in Flood Zone aN, it is undersIood that a dIaInage plan addressing a

<br />licensed by the State of Florida.
<br />If the fill material is to be used in Flood Zone "A" in connection with 8 pemlitt\:ld buldlng using stem wall
<br />construcIion. I cerIlfy that fill will be used only to III the area with.. the stem wan.
<br />If 111 material is to be used in any area, I certify that use of such fiR wi! not adversely affect adjacent
<br />properties. If use of fill is found to adversely affect adjacent properties, the owner may be cited for violating
<br />the condiIlons of the building permit issued under the atIached permit application, for IoIs less than one (1)
<br />aae which en eleYalled by fiB, an e1ag1neered drainage plan is required.
<br />If I am the AGENT FOR THE OWNER, I promise in good faith to inform the owner of the PeI1l'liIIiI19 condiIlons set forth in
<br />this aflidavit prior to commencing construction. I understand that a sepaJate permit may be required for eIectrlcaI work,
<br />plumbing, signs, wells, pools, air condltioning. gas, or other insIaIallons not specifically Included .. the appIicallon. A
<br />permit issued shall be consIrued to be a license to proceed with the work and not as authority to violate, cancel, alter, or
<br />set aside any provisions of the technical codes, nor shall issuance of a permit prevent the Building 0fIiciaI from thereafter
<br />requiring a collection of errors in plans, construction or violations of any codes. Every permit lssued shall become invalid
<br />unless the work authorized by such permit is commenced within six months of permit issuance, or if work authorized by
<br />the permit is suspended or abandoned for a period of six (6) months after the time the work is commenced. An extension
<br />may be requeslled, in writing, from the Building 0fIiciaI for a period not to exceed ninety (90) days and will demonsIrate
<br />justifiable cause for the extension. If work ceases for ninety (90) consecutlve days, the job is considered abandoned.
