<br />-: <br /> <br />Subsidence Investigation <br />Pittman Residence - Zephyrhills, Florida <br /> <br />son Project Number: 3017839 <br /> <br />EXECUTIVE SUMMARY <br /> <br />Tower Hill Insurance Group retained SDn Global Corporation (SDII) on February 8, 2008 to <br />conduct a subsidence investigation at the Pittman residence in Zephyrhills, Florida. The purpose <br />of the investigation was to determine the existence of sinkhole activity as defined by Chapter (~) <br />627.706 Florida Statutes at the residence. <br /> <br />The existence of sinkhole activity and a consequent sinkhole loss was investigated in accordance <br />with the requirements of ~627.707 Florida Statutes following standard methods. The following <br />methods were utilized in our investigation: <br /> <br />· A site inspection and homeowner interview were completed to identify local conditions, <br />damage, history of the structure, homeowner concerns, and details of the construction of <br />the house. <br /> <br />· A ground penetrating radar survey was conducted to identify anomalous subsurface areas. <br /> <br />· One test pit was excavated to observe foundation construction and geometry directly. <br /> <br />· Hand auger borings and push penetrometer soundings were advanced to determine the <br />composition and strength of subsurface materials near the foundation. <br /> <br />· Standard Penetration Test (SPT) borings were advanced to characterize the relative <br />stability of soils and deeper geologic conditions. <br /> <br />· Laboratory testing of soils was performed to identify deleterious soil types and for quality <br />control of soil and geologic materials classification. <br /> <br />· Relative floor elevation contour mapping was completed to identify anomalous floor <br />elevation trends and establish a baseline contour map of the existing floor elevation. <br /> <br />· A structural assessment was completed to identify construction methods and structural <br />deficiencies related to the potential causes of observed conditions. <br /> <br />· Mini-Cone penetrometer test (CPT) soundings were advanced to obtain continuous soil <br />strength and stability data in areas of interest <br /> <br />It is SDII's professional opinion that sinkhole activity as defined by ~627.706 Florida <br />Statutes cannot be ruled out at the Pittman residence. The SPT boring and CPT sounding <br />data do not indicate the presence of sinkhole activity. However, the presence of a ground <br />depression under the concrete slab outside the back door cannot be ruled out as being associated <br />with sinkhole activity. The depression is likely the result of a "chimney sink". <br /> <br />The observed damage on the exterior and interior of the residence is consistent with either the <br />direct result or the collateral effect of differential movement of the foundation. son recommends <br />that the sinkhole conditions be remediated using grout injection to increase the density of the <br />soils and cap the limestone. The estimated cost of remediation is approximately $36,700. <br /> <br />The following report details the basis of these findings.
