<br />PERMITTEE: Chasey Lane Townhomes, LLC <br />PROJECT: Chasey Lane Townhomes <br /> <br />PERMIT No.: 082640-081-DS/C <br /> <br />was designed under the responsible charge of a professional engineer licensed in the State of <br />Florida. The permittee must retain the service of an engineer registered in the State to <br />observe that construction of the project is in accordance with the engineering plans and <br />specifications as submitted in support of the application for this permit. [F.A.C. Rule 62- <br />555.520(3)] <br /> <br />4. A letter of clearance may be issued by this Department once the following items have been <br />submitted: <br /> <br />a. Complete and fully executed form "Certification of Construction Completion and <br />Request for Clearance to Place Pennitted PWS Components into Operation", DEP <br />Form 62-555.900(9) effective August 28, 2003 [F.A.C. RuIe 62-555.345(1)]; <br /> <br />b. The portion of record drawings showing deviations from DEP construction permit, <br />including the approved preliminary design report or drawings and specifications, if <br />there are any deviations from said permit. (Note that it is necessary to submit a copy <br />of only the portion of record drawings showing deviations and not a complete set of <br />record drawings) [F.A.e. Rule 62-555.345(1)(a)]; <br /> <br />c. Copies of satisfactory bacteriological analyses verifying that proper disinfection of the <br />water mains was conducted in accordance with 62-555.340(2)(a) through (c) and the <br />American Water Works Association (A WWA) Standard C651-99 as follows: <br /> <br />I. After reducing the total chlorine residual in the water mains to no more than <br />four milligrams per liter, a total of at least two samples - each taken on a <br />separate day and taken at least six hours apart from the other sample - shall be <br />collected at each of the locations indicated in the applicable A WW A standard, <br />. and the samples shall be analyzed for total residual chlorine and for the <br />presence of total coliform; <br /> <br />11. Samples shall be taken from the following locations: connection points to the <br />existing system and any dead end lines in the system; <br /> <br />ill. If any sample contains more than four milligrams per liter of total chlorine, the <br />sample shall be considered invalid. If any sample shows the presence of total <br />coliform, the water mains shall be redisinfected and resampled until two <br />consecutive samples at each sampling location show the absence of total <br />coliform; <br /> <br />[F.A.C. Rule 62-555.340] <br /> <br />d. Copy of a satisfactory pressure test of the distribution system performed In <br />accordance with A WW A Standards. [F.A.C. Rule 62-555.320(21)(b)(1)] <br /> <br />5. The permittee must instruct the engineer of record to request system clearance from the <br />Department within sixty (60) days of completion of construction, testing and disinfecting the <br />system.
