Payment for permit acknowledges acceptance <br /> and compliance of the conditions noted herein. The following items shall be considered: <br /> 1. Demolition and Installation must be performed by a licensed Fire Sprinkler <br /> Contractor. One has been identified on application. Demolition has occurred <br /> prior to a site visit on this date. <br /> 2. All of the installation shall be in accordance with NFPA 13, latest edition. <br /> 3. It should be noted that the new construction plans for the first floor show 20 <br /> heads to be installed as per the drawing, however; the detail notes state 19 <br /> heads to be in the new construction. Drawing and description are equal. <br /> 4. Contractor shall follow general and keyed notes as per plans. <br /> 5. The contractor shall insure that the type/model of new sprinkler heads shall be <br /> included in the required sprinkler head box if these are a change in the <br /> existing sprinkler head types in the remainder of the building. (It is not <br /> believed that these are different type heads, but must be included if they are <br /> different) <br /> 6. "As-builY' drawings shall be provided to the owner showing placement of <br /> heads in relation physical features of the completed project and on site at final <br /> inspection. <br /> Inspections Required: <br /> 1. Rough-in Inspection shall be required after new installation. <br /> 2. Final inspection shall be conducted after construction has been completed. <br /> Review and approval of the submitted plans does not relieve the contractor from the <br /> responsibility of correcting any deficiencies noted during inspections. <br /> Respec lly sub itted on 3 January 2012 by, <br /> ���� <br /> � <br /> Keith A.
