Subsidence Investigation SDII Project t�fumber:3023547 <br /> Quiros-Martinez Residence-Zephyrhills,Florida <br /> 1.0 INTRODUCTION <br /> 1.1 Background <br /> SDII Global Corporation (SD1I) was retained by Cooperativa de Seguros Multip;fes to conduct a <br /> subsidence investigation in accordance with §627.707 Florida Statutes at the (�uiros-Martinez <br /> residence in Zephyrhilis, Florida. The purpose of the .investigation was to determine the <br /> existence of sinkhote activity and a sinkhole loss as defined by §627.706 Florid�� Statutes at the <br /> residence. This report provides the results of the subsidence investigatian and SDi1's <br /> recommendations for remedial actions to stabilize the foundation. <br /> 1.2 Scope of Work <br /> The following methods were used during the investigation to detennine the exist��nce of sinkhole <br /> activity at the residence. <br /> ❑ The residence was inspected to determine the nature and extent of reporked damage and <br /> to observe existing conditions across the property. <br /> ❑ The horneowner was interviewed to obtain the timeline for the damage„ chronology of <br /> construction, ownership of the structure, and background information relaCive to potential <br /> causes of damage to the structure. <br /> ❑ Available published data were reviewed regarding generai geologic and hydrogeologic <br /> conditions in the area of the residence. <br /> G A ground penetrating radar (GPR) survey was conducted inside and outside of the <br /> residence to determine the presence of anomalous soil conditions that mig}it be indicative <br /> of karst activity, buried debris, or otherwise disturbed or unusual subsurfac;e conditions. <br /> ❑ Hand auger borings and push penetrometer soundings were advanced tc� determine the <br /> composition and strength of subsurface materials near the foundation. <br /> ❑ Standard penetration test(SPT)borings were advanced to detennine t11e relative strengths <br /> of soils and to characterize deep geologic conditions. <br /> ❑ Representative soil and geologic material collected from the hand auger and SPT borings <br /> were analyzed in ti�e laboratory to confir�n visual interpretations and furth��r characterize <br /> the properties of the materials. <br /> ❑ Three test pits were excavated to observe foundation construction and geon-ietry directly. <br /> ❑ Relative floor elevation contour mapping was completed to identify an��malous floor <br /> elevation trends and establish a baseline contour map of the existing floor elevation. <br /> ❑ A structural assessment was completed to identify construction methods and structural <br /> deficiencies related to the potential causes of observed conditions. <br /> 1-1 <br />
