OWNER—OCCUPIED REHABILITATION ASSISTANCE AGREEMENT FOR GENERAL CONTRACTORS HOME iNVESTMENT PARTNERSHIPS PROGRAM (Only if applicable if assistance funded with HOME Funds) ( THIS AGREEMENT is entered into this 4 day of November. 2013 by and between the f'asco County Board of County Commissioners(Community Development Division), (hereinafter referred to as the"County"), and Covenant Communities. lnc. {hereinafter referred to as the"Contractor"). THlS AGREEMENT IS ENTERED INTO BASED ON THE FOLIOWING FACTS: A.WHEREAS, Pasco County has been designated by the United States Department of Housing and Urban Development as a participating jurisdiction for the receipt and use of funds as p�ovided by the HOME Investment Partnerships Program ("HOME Program")as provided in 24 CFR Part 92; and B.WFiEREAS, the County has agreed to use the HOME funds to finance a Homeowner program that meets the requirement as set forth in 24 CFR Part 92 as amended by the United States Housing and Urban Development Department(HUD). C.WHEREAS, the Contractor has agreed to rehabilitate the following property: 4620 Eaale Ranae Dr.ZeohvrhilEs FL 33542 NOW,THEREFORE, the County and the Contractor do mu#ualfy agree as follows: (1) Housing Quality Standards. Once rehabilitation is completed, the p�operty will meet the property standards{HQS)as autlined in 24 CFR Part 92.251 and the Counry will ensure that the property wilf inest the requirements of its written standards, applicable local codes, rehabilitation standards, ordinances and zoning ordinances. Homes that are non-conforming but have been allowed to continue by the County or applicable city shall be allowed to participate in the pragram. (2)Rehabilita#ion Work. The work that will be done on the house sha11 be in accordance with the proposal, attached as an exhibit to this agreement. Additional work shall be approved by using a change order form. Change order forms must be signed by the contractor and the Homeowner, and approved by the County. It is estimated that the work will be campfeted according to the following schedule: Contract Signing: 11/20/13 50°/a Completion 3/Q4/14 100%Campletion 5h 8/14 The budget for this project is estimated as follows: Ti#le Costs 500.00 Rehab Work $117.580.00 Total $118.080.00 The original contract calls for the work to be c�mpleted in 150 days. (3)Other Prograrn ReQuirements.The Contractor and the County musf compiy with all applicable federal iaws and regulations as described in the HUD guidelines(24 CFR Part 92). a.
