<br /> licensed by the State af Florida.
<br /> - If the fill material is to be used in Flood Zone `A" in connectian witM a permitted building using stem wail
<br /> construc#ion, I certify that fill will be used only to dl the area within the stem w�ll.
<br /> - lf fiH material is to be used in any area, 1 ce ify that use of such fill wi11 not adversely affect adjacent
<br /> properties. If use of fill is found to adversely a ct adjacent prc�perties, the owner may be cited for vialating

<br /> the permit is suspended or abandoned for a period of six(6) manths after the time the work is commenced. An extension
<br /> may be requested, in writing, from the Building O�cial far period not to exceed ninety (94) days and wil! demonstrate
<br /> justifiable cause for the e5c#ension. If work ceases for ninety 90}consecutive days,the job is considered abandoned.
