, , I
. i

CONCLUSIONS '

Dr. Caselnova's office is located at 7209 Green Slope Drive, Zephyrhills, I
Florida. This site includes the southern half of a sin le-sto concrete masonry ;
g rY ,
commercial structure with a partial brick and stucco exterior. The structure was
built in 2002 and the southern half of the building is owned by Michael and I
Angela Caselnova. Dr. Caselnova's office manager, Betty Fulghum, stated that i
she has not noticed any distress within Dr. Caselnova's portion of the office i
building, but she was aware that distress had been noted within the northern i
section of the office building (the Pediatric Care Center of Pasco, Inc.). Limited
minor interior distress and widespread minor exterior distress was observed at the ;
Caselnova property. i

I
Hand auger borings penetrated fine sands and clayey sands with limestone
fragments within the upper 5 feet depth. No organic-rich or highly plastic clayey

materials were detected in the hand auger borings. Cone penetrometer tests
indicate that the near-surface materials range from very low to moderate bearing ;
strength. j

Electrical resistivity results are indicative of clayey sands within the upper
15 to 25 feet depth, overlying sands. No electrical evidence of the underlying ;
limestone surface was detected in the areas and depths surveyed. Ground '
penetrating radar signals interpreted as anomalous were detected along 6 of the 32
traverses.

E
The enclosed floor elevation survey documents a total interior elevation ''
difference of approximately 1.5 inches. The foundation test pits revealed the '
depth of embedment to be in accordance with current construction codes (at least ;
16 inches at TP-1 and at least 18 inches at TP-2). The structural evaluation
yielded no evidence of a significant loss of structural integrity.
4
Five standard penetration test borings were conducted at the Caselnova
property. Loose to very loose clayey sands were penetrated within the upper 6 to z
8 feet depth in four of the five SPT borings. Limestone was initially penetrated at
depths ranging from 48.5 to 69.5 feet depth in the five SPT borings. The
differential in depth to limestone is indicative of a paleosink environment
underlying the Caselnova property. A low N-value (N = 1) was recorded in I
limestone at the 60 feet depth sample interval of SPT boring B-5. A partially iri-
filled cavity, defined by a 4.5 foot weight-of-hammer condition, was penetrated in
limestone at 68.5 feet depth in boring B-2. Weight-of-rod conditions of 1.5 and 6 ;
i
I
17 '
