<br />1. FLOOR SLAB AND FINISH: A SMOOTH AND LEVEL CONCRETE FLOOR SLAB, FREE OF LEVELING PRODUCTS,
<br />WITH NOT MORE THAN ONE -EIGHTH INCH (118) VARIATION IN TEN FEET (10) AND ALL REQUIRED EXPANSION AND
<br />CONTROL JOINTS PROPERLY INSTALLED AND READY FOR TENANT -SPECIFIED FLOOR COVERINGS. ON GRADE
<br />FLOORS WILL HAVE A FUNCTIONING VAPOR RETARDER BENEATH THE SLAB.
<br />2. DEMISING WALLS: ANY INTERIOR WALL SEPARATING THE PREMISES FROM OTHER SPACE (DEMISING
<br />WALLS) SHALL BE CONSTRUCTED WITH METAL STUDS COVERED WITH DRYWALL ON BOTH SIDES, TAPED AND
<br />SANDED (READY TO PAINT), AND SHALL HAVE A MINIMUM ONE (1) HOUR FIRE RATING OR SUCH RATING AS IS
<br />REQUIRED PER APPLICABLE BUILDING CODES. LANDLORD SHALL PROVIDE DEMISING WALLS TO THE FULL
<br />HEIGHT OF THE ROOF DECK. INTERIOR SURFACES OF ALL WALLS IN THE PREMISES SHALL BE FURNISHED WITH
<br />FIVE -EIGHTS INCH (5/8) GYPSUM WALLBOARD, TAPED, SANDED SMOOTH, AND PAINTED PER TENANTS PLANS
<br />AND SPECIFICATIONS. ALL CUTTING AND PATCHING SHALL BE COMPLETED BY LANDLORD PER TENANTS PLANS

<br />C. GLASS: PER LANDLORD'S APPROVED PLAN (EXISTING)
<br />D. SECONDARY EXIT/SERVICE DOOR: DOOR SHALL BE MINIMUM T-0" WIDE & OPEN DIRECTLY TO THE
<br />EXTERIOR OR TO A HANDICAP ACCESSIBLE EXIT CORRIDOR WITH KEYED PANIC HARDWARE INSTALLED PER
<br />LOCAL BUILDING CODES AND ORDINANCES. THE LOCATION OF THE SECONDARY EXIT/SERVICE DOOR SHALL BE
<br />APPROVED BY TENANT.
<br />5. AIR-CONDITIONING AND HEATING:
<br />A. LANDLORD SHALL SUPPLY, INSTALL, DISTRIBUTE DUCT WORK FOR, BALANCE, AND START UP A NEW
<br />HEATING AND AIR-CONDITIONING UNIT/S. THE HVAC
<br />SYSTEM SHALL BE DESIGNED BY LANDLORD AND SHALL INCLUDE A DUCTED RETURN SYSTEM. THE
<br />DESIGN/LAYOUT IS TO CONFORM TO TENANTS FLOOR PLAN. FINAL DESIGN TO BE APPROVED BY TENANT. THE
<br />NEW UNITS TO BE SIZED 1 TON PER 300 SQUARE FEET. LANDLORD SHALL SUPPLY (3) THREE NEW SEPARATE
<br />PACKAGED ROOF TOP UNITS (RTUS). ONE UNIT IS TO BE DESIGNATED SOLELY FOR THE TENANTS RECEPTION
<br />AREA AND THE OTHER TWO UNITS TO BE DESIGNATED FOR THE REMAINDER OF THE OFFICE/SPACE.
<br />THE THERMOSTAT IN THE RECEPTION AREA SHALL HAVE A LOCKBOX. THE LOCKBOX KEYS MUST
<br />BE GIVEN TO ASPEN DENTAL MANAGEMENT UPON JOB COMPLETION.
<br />B. IF SMOKE DETECTORS ARE REQUIRED TO BE INSTALLED IN THE RETURN AIR DUCT BY APPLICABLE
<br />BUILDING CODE, THEN LANDLORD WILL INSTALL A VISIBLE AND AUDIBLE SMOKE DETECTOR IN THE RETURN AIR
<br />DUCT OF THE HVAC UNIT. LANDLORD SHALL SUPPLY THERMOSTATS PER TENANTS PLANS.
<br />C. LANDLORD SHALL PROVIDE TRANSFERABLE WARRANTIES ON THE CARRIER/YORK/ TRANE HEATING AND
<br />AIR-CONDITIONING SYSTEM(S). COMPRESSORS SHALL HAVE A FIVE (5) YEAR WARRANTY. THE SYSTEM AND
<br />OTHER COMPONENTS SHALL HAVE THE MANUFACTURERS STANDARD PERIOD WARRANTY, BUT IN NO EVENT
<br />LESS THAN TWELVE (12) MONTHS.
<br />D. THE HVAC UNIT CAPACITY SHALL MAINTAIN TEMPERATURE AT NOT MORE THAN SEVENTY-TWO DEGREES
<br />FAHRENHEIT (72°F) IN COOLING MODE (SUMMER) AND NOT LESS THAN SIXTY-EIGHT DEGREES FAHRENHEIT (68°F)
<br />IN HEATING MODE (WINTER). DESIGN CRITERIA FOR MECHANICAL SYSTEMS TO INCLUDE GLASS AREA AND
<br />EXPOSURE, CUSTOMER AND EMPLOYEE LOAD, LIGHTING LOAD OF THREE (3) WATTS PER SQUARE FOOT,
<br />COMPUTER AND RELATED LOAD FACTORS (PER APPLICABLE BUILDING CODES). (72°F) IN COOLING MODE IN THE
<br />OCCUPIED SETTING. LANDLORD SHALL SUPPLY THERMOSTATS PER TENANTS PLANS.
<br />E. THE SYSTEM SHALL BE COMPLETE AND FULLY FUNCTIONING AND TESTED FOR HEATING AND COOLING
<br />MODE UPON DELIVERY OF POSSESSION OF THE PREMISES TO TENANT. LANDLORD SHALL BE RESPONSIBLE FOR
<br />THE INITIAL START-UP, INSTALLATION OF NEW FILTERS PRIOR TO OPENING, TEST AND BALANCE OF THE SYSTEM.
<br />F. LANDLORD SHALL INSTALL RESTROOM EXHAUST FLUES AND REQUIRED ROOF PENETRATION/S IN
<br />ACCORDANCE WITH APPLICABLE BUILDING CODES. LANDLORD SHALL ALSO PROVIDE FRESH AIR INTAKES
<br />(ECONOMIZERS) AS REQUIRED FOR RTUS.
<br />G. THE HVAC DUCT WORK SHALL BE GALVANIZED, UNPAINTED DUCTING. HVAC DUCT WORK WILL BE
<br />INSTALLED TWELVE FEET (12) MINIMUM ABOVE FINISH FLOOR AND SHALL BE INSULATED PER INDUSTRY
<br />STANDARDS.
<br />6. RESTROOMS & PLUMBING: LANDLORD SHALL PROVIDE AS FOLLOWS:
<br />A. RESTROOMS: LANDLORD SHALL PROVIDE TWO (2) ADA COMPLIANT RESTROOMS WITH COMMON

<br />GOVERNING AGENCIES, WILL BE SUPPLIED AND INSTALLED AT LANDLORDS SOLE COST AND EXPENSE.
<br />LANDLORD WILL BE RESPONSIBLE FOR ALL DESIGN DRAWINGS FOR BOTH THE FIRE ALARM AND FIRE SPRINKLER
<br />SYSTEM. IF SUCH SYSTEM IS REQUIRED, LANDLORD SHALL INSTALL IN ACCORDANCE WITH APPLICABLE
<br />BUILDING CODES. LANDLORD SHALL SUPPLY LINES, VALVES, DROPS, AND PENDANT STYLE HEADS AS
<br />REQUIRED BY APPLICABLE BUILDING CODES AND PER TENANTS FLOOR PLAN. LANDLORD SHALL ALSO PROVIDE
<br />A BACKFLOW PREVENTER IF REQUIRED BY APPLICABLE BUILDING CODES. MONITORING DEVICES, ALARMS,
<br />PULLS, ENUNCIATOR PANELS, VISUAL AND AUDIO WARNING INDICATORS, STROBES, AND ANY OTHER REQUIRED
<br />MATERIALS, SYSTEM TESTING, INSPECTION APPROVAL BY THE LOCAL FIRE INSPECTOR AND MONTHLY
<br />MONITORING AND ALARM FEES INCLUDING TELEPHONE LINES, IF REQUIRED, ARE THE RESPONSIBILITY OF THE
<br />LANDLORD DURING THE LEASE TERM. THE FIRE RISER SHALL BE IN A LOCATION APPROVED BY TENANT.
<br />LANDLORD WILL BE RESPONSIBLE FOR ANY MONTHLY MONITORING AND ASSOCIATED FEES IN CONNECTION
<br />WITH THE OPERATION OF THE SYSTEM DURING THE LEASE TERM. FIRE ALARM SYSTEM TO BE INSTALLED PER
<br />TENANTS FLOOR PLAN OR AS REQUIRED BY APPLICABLE BUILDING CODES.
<br />7. ELECTRICAL:
<br />A. LANDLORD SHALL PROVIDE AND INSTALL A DEDICATED ELECTRICAL SERVICE WITH A MINIMUM OF TWO
<br />HUNDRED (200) AMPS VOLTAGE TO BE 120/208, 3-PHASE, 4-WIRE. ELECTRICAL SERVICE TO BE INSTALLED IN
<br />TENANTS MECHANICAL ROOM.
<br />B. LANDLORD SHALL PROVIDE AND INSTALL A METER FOR THE ELECTRICAL SERVICE DEDICATED SOLELY TO
<br />TENANT IN ACCORDANCE WITH METERING REQUIREMENTS OF THE UTILITY COMPANY. LANDLORD SHALL
<br />INSTALL THE METER IN A LOCATION ACCEPTABLE TO LOCAL UTILITY COMPANY AND TENANT. LANDLORD SHALL
<br />BE RESPONSIBLE FOR ALL INSTALLATION RELATED METER FEES.
<br />C. LANDLORD TO PROVIDE AND INSTALL A 200 AMP 1201208 3-PHASE ELECTRICAL SERVICE WITH MAIN
<br />DISCONNECT USING SQUARE D PANELS, ONE (1) 225 AMP 42 SPACE PANEL WITH MAIN DISCONNECT AND (1)100
<br />AMP 24 SPACE SUB PANEL INSIDE TENANTS MECHANICAL ROOM INCLUDING ALL SQUARE D BREAKERS.
<br />CONDUIT AND WIRE SIZES PER NATIONAL ELECTRICAL CODE. ALL WIRE AND BUS SHALL BE COPPER. ALL
<br />PANELS SHALL BE LOCATED IN ACCORDANCE WITH TENANTS PLANS. FINAL LOCATION MUST BE VERIFIED WITH
<br />TENANT PRIOR TO INSTALLATION.
<br />D. LIFE SAFETY SYSTEMS (I.E. SMOKE ALARMS AND FIRE DETECTION SYSTEMS) ARE ALSO THE
<br />RESPONSIBILITY OF THE LANDLORD, IF REQUIRED BY CODE. LANDLORD WILL BE RESPONSIBLE FOR ANY
<br />MONTHLY MONITORING AND ASSOCIATED FEES DURING THE LEASE TERM.
<br />E. LANDLORD SHALL PROVIDE EXIT SIGNS & EMERGENCY LIGHTS AS REQUIRED BY APPLICABLE BUILDING
<br />CODES.
<br />F. LANDLORD SHALL PROVIDE AND INSTALL 2'X2' & 2'X4' LAY -IN LIGHT FIXTURES WITH ACRYLIC LENSES TO
<br />CURRENT CODE BASED ON 1 FIXTURE PER 60 SF IN ACCORDANCE WITH TENANTS FLOOR PLAN. LIGHTING
<br />SPECIFICATION WILL BE A COMBINATION OF 2,3 & FOUR LAMP FIXTURES WITH ELECTRONIC BALLASTS AND
<br />SHALL BE LIGHTOLIER OR EQUAL. ALL SWITCHING SHALL BE PER TENANTS FLOOR PLAN.
<br />G. LANDLORD SHALL INSTALL ONE HUNDRED TWENTY (120) VOLT DUPLEX OUTLETS PER LOCAL BUILDING
<br />CODES AROUND THE PERIMETER EXTERIOR WALLS OF THE DEMISED PREMISES.
<br />H. LANDLORD SHALL PROVIDE A MINIMUM OF ONE (1) ONE HUNDRED TWENTY (120) VOLT DUPLEX OUTLET
<br />WITH GROUND FAULT INTERRUPTER (GFI) IN EACH RESTROOM AT LOCATIONS PER TENANTS PLANS AND
<br />SPECIFICATIONS AND LOCAL BUILDING CODES.
<br />1. LANDLORD SHALL PROVIDE POWER FOR EXTERIOR SIGNAGE AS REQUIRED PER TENANTS SPECIFIED
<br />LOCATIONS AND POWER REQUIREMENTS. LANDLORD IS TO PROVIDE A TIME CLOCKS FOR THE EXTERIOR
<br />SIGNAGE INSTALLED NEXT TO THE ELECTRICAL PANELS IN THE TENANTS MECHANICAL ROOM.
<br />J. LANDLORD SHALL PROVIDE ONE (1) ONE HUNDRED TWENTY (120) VOLT QUADPLEX OUTLET NEXT TO
<br />ELECTRICAL PANEL FOR TENANTS PHONE SYSTEM PER EXISTING LOCATION.
<br />K. LANDLORD SHALL FURNISH AND INSTALL A TWO INCH (2) CONDUIT LABELED FOR CABLE TV COMPANY USE
<br />ONLY FROM TENANTS MECHANICAL ROOM TO THE NEAREST UTILITY POLE ON SITE.
<br />8. CEILING:
<br />SUSPENDED ACOUSTICAL 2' X 4' CEILING TILE AND GRID SYSTEM WITH ARMSTRONG SECOND LOOK #2767
<br />INSTALLED PER TENANTS FLOOR PLAN. CEILINGS WILL BE INSTALLED AFTER THE INTERIOR WALLS ARE FRAMED.
<br />9. UTILITIES:
<br />LANDLORD TO PROVIDE ELECTRICAL, GAS A 100 POUND PROPANE GAS TANK (TO BE INSTALLED AT THE BACK
<br />OF THE BUILDING / PLAZA) ANDAND WATER SERVICE TO THE PREMISES AND PAY ALL CONNECTION
<br />AND HOOKUP FEES ASSOCIATED. WATER SERVICE TO BE 1 MINIMUM. GAS TO BE SIZED PER TENANT LOAD
<br />REQUIREMENTS. ELECTRICAL PER SECTION 10 ABOVE. ALL ACCOUNTS ARE TO BE SET UP IN LANDLORDS NAME
<br />AND WILL BE TRANSFERRED TO TENANT UPON DELIVERY OF THE SPACE.
<br />10. OTHER:
<br />A. LEASE SPACE SHALL HAVE NO HAZARDOUS MATERIALS IN ANY FORM, INCLUDING, BUT NOT LIMITED TO
<br />ASBESTOS, ASBESTOS PRODUCTS, POLYCHLORINATED BIPHENYL (PCB) OR OTHER TOXIC SUBSTANCES.
<br />B. LEASE SPACE SHALL BE CLEARED OF ANY STORED MATERIALS, FIXTURES, FURNITURE, MILLWORK OR
<br />EQUIPMENT.
<br />C. PATH FROM PARKING AREA TO FRONT DOOR SHALL BE HANDICAP ACCESSIBLE.
<br />D. VANILLA SHELL SHALL MEET ALL LOCAL BUILDING CODES, ADA REQUIREMENTS, AND ZONING ORDINANCES.
<br />E. ROOFING SYSTEM ABOVE LEASE SPACE SHALL BE WATERTIGHT AND UNDER WARRANTEE. THE COST OF ALL
<br />REQUIRED REPAIRS TO MAINTAIN THE ROOFING SYSTEM IN A WATERTIGHT CONDITION, AND ANY ASSOCIATED
<br />LOSS BY TENANT, SHALL BE PAID BY THE LANDLORD.
<br />F. PARKING LOT WILL BE STRIPED, ADEQUATE
<br />HANDICAP PARKING AND IN GOOD REPAIR.
<br />SCOPE OF WORK
<br />GENERAL NOTES:
<br />1. ALL WORK SHALL BE DONE IN STRICT ACCORDANCE WITH ALL APPLICABLE LOCAL,
<br />STATE AND FEDERAL BUILDING CODES AND/OR REGULATIONS.
<br />2. ALL WORK SHALL BE DONE IN A MANNER CONSISTENT WITH THE HIGHEST
<br />STANDARDS OF THE RESPECTIVE TRADES.
<br />3. THE CONTRACTOR SHALL VISIT THE SITE AND FAMILARIZE HIMSELF WITH THE
<br />EXISTING CONDITIONS BEFORE BIDDING.
<br />4. THE CONTRACTOR SHALL VERIFY ALL FIELD DIMENSIONS BEFORE PROCEEDING
<br />WITH THE WORK. IN THE EVENT OF A DISCREPANCY, THE CONTRACTOR SHALL
<br />NOTIFY ASPEN DENTAL MANGMENT INCORPORATED (ADMI) IMMEDIATELY.
<br />5. THE CONTRACTOR SHALL ABIDE BY ALL REQUIREMENTS OF THE OWNER WITH
<br />RESPECT TO CONSTRUCTION SCHEDULING, COORDINATION, TEMPORARY
<br />CONSTRUCTION UTILITIES, ECT.
<br />6. ALL MATERIALS, PRODUCTS AND UNITS SHALL BE INSTALLED PER
<br />MANUFACTURERS RECOMMENDATIONS AND INSTRUCTIONS. ASPEN DENTAL
<br />SHALL BE NOTIFICED IMMEDIATELY OF ANY DISCREPANCIES.
<br />7. INSTALLATION OF ALL MATERIALS AND/OR UNITS TO BE SELECTED BY, SUPPLIED BY,
<br />AND/OR INSTALLED BY THE OWNER SHALL BE SCHEDULED AND COORDINATED BY
<br />THE CONTRACTOR TO MAINTAIN THE CONSTRUCTION SCHEDULE. PRIOR TO THE
<br />COMMENCEMENT OF THE WORK, THE CONTRACTOR SHALL NOTIFY THE OWNER OF
<br />ALL QUANTITIES OF OWNER SUPPLIES MATERIALS AND/OR UNITS NOT SPECIFICALLY
<br />CALLED OUT IN THESE CONSTRUCTION DOCUMENTS. THE CONTRACTOR SHALL
<br />NOTIFY THE OWNER OF REQUIRED DELIVERY DATES OF OWNER SUPPLIED
<br />MATERIALS AND UNITS.
<br />8. ALL FINISH PAINT SHALL BE APPLIED OVER A COMPATIBLE FACTORY OR FIELD
<br />APPLIED PRIMER.
<br />9. THE CONTRACTOR SHALL PROTECT ALL EXISTING AND ADJACENT AREAS AT ALL
<br />TIMES DURING CONSTRUCTION. ANY AREA DAMAGES OR AFFECTED BY
<br />CONSTRUCTION SHALL BE PATCHED, REPAIRED, OR REPLACED AS REQUIRED TO
<br />MATCH EXISTING OR ADJACENT AREAS AT THE CONTRACTORS EXPENSE.
<br />10. THE CONTRACTOR SHALL YIELD TO THE OWNER AND THEIR VISITORS AT ALL TIMES.
<br />11. THE CONTRACTOR SHALL NOT DISRUPT THE BUILDING OR OPERATIONS WITHOUT
<br />PRIOR SCHEDULING AND APPROVAL FROM THE OWNER/LANDLORD.
<br />12. THE ARCHITECT ASSUMES NO RESPONSIBILITY FOR ANY DEVIATION FROM THE
<br />FINAL CONSTRUCTION DOCUMENTS, NO MATTER WHAT THE CAUSE, UNLESS A
<br />WRITTEN DIRECTIVE IS GIVEN BY THE ARCHITECT.
<br />13. IF CONFLICT OCCURS ON THESE CONSTRUCION DOCUMENTS AND/OR THE
<br />SPECIFICATIONS, THE CONTRACTOR SHALL BID THE HIGHER QUALITY AND/OR

<br />AS REQUIRED. ROOF WORK SHALL BE PERFORMED BY LANDLORDS ROOFING

<br />22. THE CONTRACTOR SHALL REMOVE ALL RUBBISH AND WASTE MATERIAL ON A DAILY
<br />BASIS AND KEEP THE JOB SITE AND ALL EGRESS EXIT PATHWAYS BROOM CLEAN AT
<br />ALL TIMES. ALL WASTE MATERIAL SHALL BE DISPOSED OF PROPERLY.
