Approval is contingent upon no <br />objection to the permit being received by the District within the time <br />frames described below. <br /> <br />The following statements describe procedures established by Florida law. <br />should you or any other person disagree with the District's decision <br />regarding this permit. State law and District rules provide that any <br />person who is substantially affected by this decision for approval may <br />petition for an administrative hearing in accordance with Section 120.569, <br />Florida Statutes (F.S.), and Part V of Chapter 40D-1, Florida <br />Administrative Code. <br /> <br />A request for hearing must be filed with (received by) the Agency Clerk of <br />the District within 14 days after the date of receipt of this notice. <br /> <br />Failure to file a request for hearing within the 14-day period constitutes <br />a waiver of the right such person has to request a hearing under Section <br />120.569, F.S. When the actual date of receipt of this notice cannot be <br />determined, receipt is deemed to be the fifth day after the date on which <br />this letter is postmarked. <br /> <br />The enclosed approved construction plans are part of the permit, and <br />construction must be in accordance with these plans. <br /> <br />If you have questions concerning the permit, please contact David Z.
