(HBI) is pleased to present this proposal for sinkhole remediation by subsurface
grouting. This proposal is based upon Central Florida Testing Laboratories, Inc.'s (CFTL)
Comprehensive Geotechnical Investigation.

PROBLEM DESCRIPTION

According to the Comprehensive Geotechnical Investigation, the home is bearing upon soils that are
being affected by deep sinkhole activity. In accordance with standard engineering and construction
practices and in accordance with the geotechnical engineer, HBI will be performing a subsurface
grouting program to stabilize the deep sinkhole conditions.

GROUTING PROGRAM

We follow CFTL's recommendations and scope to stabilize the sinkhole conditions by using a
subsurface grouting program. This grouting program involves the injection of a sand, cement-fly ash
grout under high pressure to compact and displace soils. In addition, the grout will tend to plug any
breaches in the soil rock interface and minimize the possibility of future raveling of soils into
solution cavities.

The grout point locations indicated on the attached sketch have been located with similar projects in
mind and as per CFTL' s recommendations. These are all preliminary and may be changed either
before or during the grouting program as directed by the soils engineer.

+--

California. Colorado. Florida.
