Subsurface Grouting
Center for Radiation Oncology
Zephyrhills, FL

March 18, 2008
Page 2

GROUTING WORK PLAN

Drilling equipment will install nominal two to three inch diameter flush joint steel casing (grout
pipe) at grout point locations as shown on the sketch. The casing will extend to anticipated depths
on the order of approximately 55 to 85 feet beneath the ground surface. The intent is to intercept the
limestone/soil interface, and continue drilling until firm limestone is encountered. HBI will angle
their grout pipes as requested to maximize program effectiveness by treating under the interior of the
building.

The grouting will begin at the limestone layer. At this depth the grout will be pumped at pressures in
the range of 50 psi to 300 psi. The higher pressures are necessary in order for the grout to seek loose
areas and voids that exist in the limestone layer. At the limestone layer, the grout will form a mass
or bulb that will tend to travel along the limestone/soil interface.

Upon successfully treating the limestone layer, indicated by a pressure buildup, the grout pipe is
semi-continuously withdrawn in one to two foot intervals. At each interval, the grout is pumped to
displace and densify the surrounding soils. Again, the grout tends to expand in a mass or bulb. This
expansion will displace, densify and collapse piping voids and channels that may exist in the
overburden soils. The pumping continues at each interval until a satisfactory buildup of pressure is
attained.

The injection of the subsurface grout at grout point locations is controlled by both volume and
pressure. The elevation of adjacent ground surfaces and structures are monitored by HBI during the
grouting process.

SITE ACCESS

The grout points will be installed to minimize the amount of exterior and interior damage to the
landscaping, trees, lawn area, etc. However, HBI shall not be liable or responsible for damage that
occurs due to normal performance of the work. This includes damage to the landscaping. All
vehicles, breakables, etc., should be removed from our work area prior to our mobilization. A brief
site meeting prior to our mobilization would be helpful in clarifying our access requirements and any
potential problem areas.

SCOPE

In HBI's experience with similar sinkhole profiles, the grouting scope, and a similar project, the
area yields an estimated quantity of approximately 400 to 600 cubic yards of compaction grout
injected through approximately 38 primary grout points. The cost of the compaction grouting
program should be between $101,320 and $152,440. Our Schedule of Prices summarizes our
cost estimate.

IIA~
---

A Keller Company
