<br />;:. <br />l'Jaliml\ fide Insuranct.( - Center for Radiation Oncology, 7315 Green Slope Drive, Zephyrhills, FL <br />Lab No. 190442 <br /> <br />Page 5 <br /> <br />TOPo2raphv <br />A review of the 1990 USGS topographic quadrangle titled Dade City, FL shows <br />that the site is located within a relatively hilly portion of Pasco County. The map <br />also shows that the site is located along the west side of a large hill that ranges in <br />elevation from approximately 175 feet to 85 feet National Geodetic Vertical <br />Datum (NGVD). The map shows that the natural ground where the site is located <br />is between 126 feet and 116 feet NGVD, which is an approximate 10-foot slope <br />across the property from west to east. A portion of the topographic map is <br />included in this report as Figure 6. <br /> <br />Field observations indicate that the subject site has been reworked, filled and <br />graded to produce a level building site and that a very steep slope has been <br />formed directly adjacent to the west side of the building (beginning less than 5 <br />feet from the west wall) that slopes down to an existing drainage basin. <br /> <br />Potentiometric Surface <br />A review of a recent (September 2005) map depicting the potentiometric surface <br />of the upper Floridan aquifer distributed by the Southwest Florida Water <br />Management District (SWFWMD) and the USGS showed that the potentiometric <br />surface of the upper Floridan aquifer to be approximately 70 feet NGVD or <br />approximately 50 feet below land surface, indicating that a downward hydraulic <br />gradient does exist at this site. <br /> <br />Aerial Photo2raphv <br />A review of the available historical aerial photographs dating back to 1942 of <br />Pasco County from the USGS, P ALMM, and the Pasco County Property <br />Appraiser was conducted. These early aerials show the surrounding area as <br />partially developed with cleared land, early roads and citrus groves located to the <br />east of the site location. These aerial photos found that the site location was <br />mostly undeveloped in 1941 and 1957 with abundant trees and with the northeast <br />portion being a part of a citrus grove. The aerial photographs did not show any <br />obvious land surface features of concern, such as wetlands, circular ponds, or <br />mining operations, on the lot where the Center for Radiation Oncology building <br />was later constructed. A copy of the 1957 aerial photograph showing the area <br />surrounding the site location was included in this report as Figure 5. A recent <br />color aerial photograph from 2006 showing development of the nearby area and <br />the location of the Center for Radiation Oncology building is included as Figure <br />4. <br /> <br />Floor Elevation Survey <br /> <br />The elevation survey of the interior floor found a total elevation differential of 2.4 <br />inches across the main floor of the building between the highest area along the <br />eastern portion of the building (in the waiting areas, the reception area and in the <br />east bathroom) and the lowest area along the western portion of the building <br />(west-central office and laboratory). <br /> <br />The raised rear concrete deck generally sloped away from the building's west <br />elevation, which would be typical to allow for drainage. The front porch <br />generally sloped away from the building, which is also typical to allow for <br />drainage. <br /> <br />The American Concrete Institute (ACI) Craftsman Series-Slab on Grade Table 5.2 <br />lists the floor finish tolerance for conventional bull floated slabs to have a <br /> <br />Central Florida Testing Laboratories, Inc. <br />12625 - 40th Street North, Clearwater, FL 33762 <br />
