There were several hairline to 1/32"
wide cracks below and extending from the lower corners of the west
windows of the offices, lab and physics rooms. There were a couple
cracks in the precast concrete windowsills that appeared to occur at joint
seams.

· West Elevation - There were multiple hairline cracks in the lower mortar
joints of the west face of the masonry wall that encloses the air
conditioning units that seems older in nature. There was up to 1/8"
separation cracking at the joint of the north masonry wall and the
northwest corner of the building. There were several hairline cracks in the
stem wall and lower portion of the wall that appear to be older in nature.
There was a 1/16" wide vertical crack along the exterior wall of the block
room. There were multiple hairline to 1/32" wide cracks in most of the
mortar joint of the raised rear deck, all of which appeared to be older in
nature. It was observed that the northern portion of this wall steps out at
the base and appears to have been thickened by design, possibly due to the
extra load of the lead lined radiation room or because of the significant
amount of fill within the stem wall on this side of the building. It also
appears that some type of grade beam has been added at the base of this
portion of the wall to provide additional support. It is not known if this
was due to a slope stability issue that arose during construction of the
building or was added at a later time.

Our inspection of the interior of the building noted the following.

· Foyer - There was a 3/32" wide crack in several of the floor tiles. There
was up to 1/16"cracking in a small decorative glass block wall knee
located to the south of the entrance that overlies the tile crack.

· Dark Room - There was hairline separation cracking along the doorframe
of the south utility room door.

· Central Hallway - There was deformation and cracking in a ceiling panel
near a light fixture.

· Simulation Room - There was evidence of water intrusion (water staining)
along the lower portion of the east wall.

For reference, we have included color photographs of the building and some of
the damaged areas, an area map showing the site location (Figure 1.) and a site
schematic (Figure 2.) showing pertinent features and our testing locations.

Review of Published Data

Soil Survey
A review of the Natural Resource Conservation Service (NRCS) publication titled
Soil Survey of Pasco County, Florida, issued by the U.S. Department of
Agriculture (USDA) in 1982 was used to determine the type soils that are found
beneath subject site. The survey lists the soils found beneath the subject property
and the immediate surrounding area as being Arredondo fine sand. The survey
lists Arredondo fine sand as a nearly level to gently sloping, well-drained soil
found as irregular shaped areas on the uplands. The slopes are smooth to concave
and typically have 0% t05 5% slope. The water table is more than 6.0 feet deep.

Central Florida Testing Laboratories, Inc.
12625 - 40th Street North, Clearwater, FL 33762
