Highway 301, and west of Highland Loop as shown on the <br />attached location map (Figure 1.). <br /> <br />According information obtained from the Pasco County Property Appraisers' <br />online database, the building was constructed in 1998. <br /> <br />A representative for the Center for Radiation Oncology, Ms. Sharon Hoffman, <br />was present during our inspection of the building on March 13, 2007. It is our <br />understanding that the cause of the claim was from the recommendation of the <br />Structure Condition Assessment by Bracken Engineering and the ongoing damage <br />of the cracking in the floor of the foyer. <br /> <br />Our inspection exterior of the building noted the following. For reference to <br />describing damage and site features, the building is located on the west side of <br />Green Slope Drive and the front of the building faces east. <br /> <br />· North Elevation - There was a hairline horizontal crack near the northeast <br />comer. There was a hairline to 1/16" vertical crack at the upper portion of <br />the wall, near the center of the wall. There were a few minor hairline <br />vertical cracks in the stucco texture along the exterior wall of the radiation <br />room. There was a newly installed retention wall approximately 7 to 9 <br />feet high installed at the north neighboring property along the northern <br />property line approximately 25 feet north of the building. The retention <br />wall appeared to be in good condition. There were several small hairline <br />cracks along both sides of the masonry wall that encloses the area where <br />the air conditioning units are located. <br /> <br />· South Elevation - There was a hairline crack in the precast concrete <br />windowsill of the southwest office that appeared to occur at a joint seam. <br />There was a hairline crack at the lower right comer of the window of the <br />southwest office. There was a hairline vertical crack near the middle of <br />the elevation. <br /> <br />· East Elevation - There was a few hairline cracks in the curved decorative <br />wall near the front entrance. There was up to 1/8" separation along the <br />vertical joint of the east wall and the decorative (curved) stucco/glass <br />block wall near the entrance. There was hairline step cracking along the <br />lower portion of the east exterior wall of the Simulation room, near the <br />location of the interior water staining and near the northeast comer of the <br />building, all of which seemed older in nature. There was apparent <br />corrosion along the vertical control joints in the wall and minor hairline <br />cracking at the control joints.
