Perform hand auger borings around the perimeter of the structure to better <br />characterize the shallow soil conditions and to identify any deleterious <br />materials such as buried debris, organic soils, or expansive clays that may <br />be present. <br /> <br />. Perform static cone penetrometer tests around the perimeter of the <br />structure to better characterize the penetration resistance of the shallow <br />soils encountered. <br /> <br />. Conduct test pit excavations to determine the type of foundation, to <br />measure its dimensions and the depth of embedment. <br /> <br />. Perform four (4) standard penetration test (SPT) borings to determine the <br />stability of the soil profile relative to the possible presence of sinkhole <br />activity. <br /> <br />. Issue a report of our findings including our opinion as to the cause(s) of <br />the damage to the building. <br /> <br />Previous Work <br /> <br />A copy of a previously completed engineering report was furnished to our office <br />for review. Bracken Engineering, Inc. completed a report of findings titled <br />"Limited Structure Condition Assessment: Center for Radiation Oncology" at the <br />subject property for Nationwide Insurance in February 2007 (File No. 307-007) <br />that summarized the their limited structure condition assessment and presented an <br />opinion as to the cause of damage to the building. The Bracken Limited Structure <br />Condition Assessment consisted of a cursory, non-destructive visual evaluation <br />with observations documented with photographs. It was the opinion of Bracken <br />Engineering, based on their observations, that the interior wall damages and the <br />water intrusion observed and reported was directly attributable to the stucco <br />control joint deficiencies; that the lack of a protective coating atop the steel <br />control joint served to promote degradation of the steel and subsequent <br />delamination of the stucco, thus promoting water intrusion to the interstitial wall <br />cavities; that the pronounced crack within the foyer floor slab was the result of a <br />shrinkage crack aggravated by differential movement of the structure; that while <br />the cause of the damage was found to be soil displacement, the full cause and <br />extent of the soil displacement remains unidentified. Bracken Engineering <br />recommended that a geotechnical engineer be consulted so as to identify any <br />subsurface anomalies and to develop a subsurface remedial plan to arrest any <br />further differential displacements as deemed necessary. <br /> <br />Site Description and Inspection <br /> <br />The subject structure is a one story, slab on grade, masonry structure located in <br />southeastern Pasco County in Zephyrhills, Florida. The property is in Section 34 <br />of Township 25 south, Range 21 east. <br /> <br />According to the Pasco County property appraiser, the property is owned by <br />Center For Radiation Oncology of Zephyrhills Inc, and the legal description of the <br />property is as follows. <br /> <br />Central Florida Testing Laboratories, Inc. <br />12625 - 40th Street North, Clearwater, FL 33762 <br />
