OWNER—OCCUPtED REHABILITATtON ASSISTANCE AGREEMENT <br /> FOR GENERAL CONTRACTORS <br /> HOME INVESTMENT PARTNERSHIPS PROGRAM <br /> (Only if applicable if assistance funded with HOME Funds) <br /> � THIS AGREEMENT is entered into this 4 day of November, 2013 by and between the <br /> Pasco County Board of County Commissioners(Community Development Divisian}, <br /> (hereinafter refe�red to as the"County'�, and Covenant Communities, Inc. (hereinafter <br /> referred to as the"Contractor"j. <br /> THIS AGREEMENT IS ENTERED 1NT0 BASED ON THE FOLLOWING FACTS: <br /> A.WHEREAS, Pasco County has been designated by the United States Department of Housing <br /> and Urban Development as a participating jurisdiction for the�eceipt and use of funds as provided <br /> by the HOME investrnent Partnerships Program ("HOME Program"}as provided in 24 CFR Part <br /> 92; and <br /> B.WHEREAS, the County has agreed to use the HOME funds to finance a Homeowner program <br /> that meets the requirement as set#orth in 24 CFR Part 92 as amended by the United States <br /> Housing and tJrban Development Departmen#(HUD). <br /> C.WHEREAS, the Contractor has agreed to rehabifitate the following property: <br /> 4702 Eaqle Ranqe Dr. Zephvrhills. FL 33542 <br /> NOW,THEREFORE, the County and the Contractor do mutually agree as follaws: <br /> {1) Housing Quality Standards.Once rehabilitation is compieted,the property will meet the <br /> property standards(HQS) as outlined in 24 CFR Part 92.251 and the County will ensure that the <br /> property will meet the�equirements of its written standards, applicable local codes, rehabilrtation <br /> standards, ordinances and zoning o�dinances. Homes that are non-conforming but have been <br /> aflowed to continus by the County or applicable city shali be allowed to participate in the program. <br /> (2)Rehabilitation Work. The work that wil{be done on the house sha0 be in accordance with the <br /> proposal, attached as an exhibit to this agreement. Additional work shall be approved by using a <br /> change order form. Change order forms must be signed by the contractor and the Homeowner, <br /> and approved by the County. <br /> It is estimated that the work will be completed according to the following schedule: <br /> Contract Signing: 11l20/13 <br /> 50°� Completion 3/04/14 <br /> 100%Completian 5/18/14 <br /> The budget for this project is estEmated as follows: <br /> Title Costs 500.00 <br /> Rehab Work $111.580.00 <br /> Total $112.080.00 <br /> The original contract calls for the work to be completed in 150 days. <br /> (3)Other Program Requirements.The Contractor and the County must comply with a(I <br /> applicable federal laws and regulations as described in the FEUD guidelines(24 C�R Part 92). <br /> a. No person in the United State shall on the grounds <br /> of race, color, riational origin, religion, handicap or sex 6e excluded from participation in, be <br /> denied the benefits of, or be subjected to discrimination under any program or activity <br /> funded in whole or in part with HOME funds.
