OWNER—OCCUPIED REHABILITATION ASSISTANCE A(3REEMENT <br /> FOR GENERAl.CONTRACTORS <br /> HOME INVESTM�MT PARTNERSHtPS PROGRAM <br /> (Only if applicable if assistance funded with HOME Funds) <br /> � THIS AGREEMENT is entered into this 4 day of November, 2013 by and befinreen the <br /> Pasco County Board of County Commissioners(Community Development Division), <br /> (hereinafter referred to as the"County"), and Covenant Communities. Inc. (hereinafter <br /> referred to as the"Contractor"). <br /> THIS AGREEMENT IS ENTERED INTO BASED ON THE FOLLOWING FACTS: <br /> A. WHEREAS, Pasco County has been designated by the United States Department of Housing <br /> and Ur'ban Development as a participating jurisdiction for ti�e receipt and use of funds as provided <br /> by the HOME Investment Partnerships Program {"HOME Program")as provided in 24 CFR Part <br /> 92;and <br /> B.WHEREAS, the County has agreed to use the HOME funds to finance a Homeowner program <br /> that meets the requiremen#as set farth in 24 CFR Part 92 as amended by the United States <br /> Housing and Urban Development Department(HUD). <br /> C.WHEREAS, the Contractor has agreed to rehabilitate fhe following property: <br /> 4549 Eaqle Ranae Dr,Zeahyrhills. FL 33542 <br /> NOW,THEREFORE, the County and the Contractor do mutuaily agree as foliows: <br /> (1)Housing Quality Standards.Once rehabilitation is completed, ths property wi(I meet the <br /> property standards(HQS)as outiined in 24 CFR Part 92.251 and the County wiil ensure that the <br /> property wilf ineet the requirements of its written standards,applicable focal codes, rehabilita#ion <br /> standards, ordinances and zoning ordinances. Homes that are non-conforming but have been <br /> allowed to continue by fhe County or applicable city shall be allowed to participate in the program. <br /> {2) Rehabilitation Work.The work that will be done an the house shatl be in accordance with the <br /> proposal, attached as an exhibit to this agreement. Additionaf work shall be approved by using a <br /> change order form. Change order forms must be signed by the contractor and the Homeowner, <br /> and appraved by the County. <br /> !t is estimated that the work will be compteted according to the following schedule: <br /> Contract Signing: 1 i/20/13 <br /> 50% Completion 3/04/14 <br /> 740%Completion 5/18/14 <br /> The budge#for this project is estimated as follows: <br /> Title Costs 500.00 <br /> Rehab Wotic $111.580.Q0 <br /> Tvtal $112.080.00 <br /> The original contract cafls for the worlc to be completed in 150 days. <br /> (3)Other Program Requirements.The Contractor and the County must comply with all <br /> applicable federal laws and regulations as described in the HUD guidelines(24 CFR Part 92). <br /> a. No person in the United State shall on the grounds <br /> oP race, cofor, national origin, religion, handicap or sex be excluded from participation in, be <br /> denied the benefits of, or be subjected to discrimination under any program or activity <br /> funded in whole or in part with HOME funds.
