OWNER—OCCUPIED REHABILITATION ASSISTANCE A(3REEMENT FOR GENERAl.CONTRACTORS HOME INVESTM�MT PARTNERSHtPS PROGRAM (Only if applicable if assistance funded with HOME Funds) � THIS AGREEMENT is entered into this 4 day of November, 2013 by and befinreen the Pasco County Board of County Commissioners(Community Development Division), (hereinafter referred to as the"County"), and Covenant Communities. Inc. (hereinafter referred to as the"Contractor"). THIS AGREEMENT IS ENTERED INTO BASED ON THE FOLLOWING FACTS: A. WHEREAS, Pasco County has been designated by the United States Department of Housing and Ur'ban Development as a participating jurisdiction for ti�e receipt and use of funds as provided by the HOME Investment Partnerships Program {"HOME Program")as provided in 24 CFR Part 92;and B.WHEREAS, the County has agreed to use the HOME funds to finance a Homeowner program that meets the requiremen#as set farth in 24 CFR Part 92 as amended by the United States Housing and Urban Development Department(HUD). C.WHEREAS, the Contractor has agreed to rehabilitate fhe following property: 4549 Eaqle Ranae Dr,Zeahyrhills. FL 33542 NOW,THEREFORE, the County and the Contractor do mutuaily agree as foliows: (1)Housing Quality Standards.Once rehabilitation is completed, ths property wi(I meet the property standards(HQS)as outiined in 24 CFR Part 92.251 and the County wiil ensure that the property wilf ineet the requirements of its written standards,applicable focal codes, rehabilita#ion standards, ordinances and zoning ordinances. Homes that are non-conforming but have been allowed to continue by fhe County or applicable city shall be allowed to participate in the program. {2) Rehabilitation Work.The work that will be done an the house shatl be in accordance with the proposal, attached as an exhibit to this agreement. Additionaf work shall be approved by using a change order form. Change order forms must be signed by the contractor and the Homeowner, and appraved by the County. !t is estimated that the work will be compteted according to the following schedule: Contract Signing: 1 i/20/13 50% Completion 3/04/14 740%Completion 5/18/14 The budge#for this project is estimated as follows: Title Costs 500.00 Rehab Wotic $111.580.Q0 Tvtal $112.080.00 The original contract cafls for the worlc to be completed in 150 days. (3)Other Program Requirements.The Contractor and the County must comply with all applicable federal laws and regulations as described in the HUD guidelines(24 CFR Part 92). a.
